摘要
背景代谢综合征(MS)能显著增加心血管疾病的发病风险,其单个组分对心血管疾病发生的影响尚存在争议。目的评估MS各组分与心血管疾病的联系,寻找其中最主要的心血管疾病危险组分。方法研究对象来自江苏省多代谢异常和MS综合防治研究队列的资料,具有完整分析数据的对象共3598人(男:1451人,女:2147人)。MS诊断采用美国胆固醇教育计划成人治疗组第3次指南标准,χ2检验和COX回归模型用于估计MS及其各个组分与心血管疾病的联系。结果在MS的5个组分中,仅血压和三酰甘油在两性中随年龄有增高趋势(P<0.05);调整年龄、性别、吸烟、饮酒和心血管病家族史以及将MS组分代入模型互为调整,结果均显示仅血压升高和高三酰甘油是心血管疾病独立的危险因素(P<0.05),其中血压与心血管疾病的相对危险比最大,调整后相对危险比为2.34(95%CI:1.41~3.90);仅血压升高者发生心血管疾病的危险性高于无血压升高的MS患者。结论MS的各组分预测心血管疾病的能力不同,其中血压是MS中对心血管疾病影响最大的组分。
Background Metabolic syndrome (MS) significantly increases the risk of cardiovascular disease (CVD), but the association between each individual component and CVD remains uncertain. Objective To evaluate the association between each component of MS and CVD and try to find the most important one. Methods Subjects were recruited from a cohort study on the prevention of multiple metabolic disorders in Jiangsu province, China. A toial of 3598( 1451 men and 2147 women) subjects were recruited and follow up for 6.3 years. MS was defined using the National Cholesterol Education Program Adult Treatment Panel Ⅲ report criteria. Z2 Test and Cox proportional hazard regression model were used to examine the association of each component of the MS and the risk of CVD. Results Only blood .pressure and triglyceride were increased with increasing age in both sexes. After adjustment of age, sex, cigarettes smoking, alcohol comsumption, family history of CVD and further adjust- ment among components each other, both results showed that only elevated blood pressure and high triglyceride were statistically significant associated with CVD(P〈0.05) and elevated blood pressure had the greatest relative risk [aRR=2.34{95% CI: 1.41-3.90)]. MS subjects with elevated blood pressure had greater CVD risk than the MS without elevation of blood pressure. Conclusion The impact of various risk factors on CVD is different, with blood pressure being the most important.
