摘要
目的研究目前北京市社区卫生服务双向转诊现况,分析其可能存在的原因并提出对策及建议。方法收集北京市18个区县的2 082家社区卫生服务机构2008年3—9月的诊疗人次数、上转患者人次数和综合医院下转患者人次数,计算各区县社区卫生服务机构上转率和综合医院下转率。分别比较全市18个区县的社区卫生服务机构上转率与综合医院下转率、城区与郊区社区卫生服务机构的上转率、城区与郊区综合医院的下转率之间是否存在差距。结果全市18个区县的社区卫生服务机构上转率与综合医院下转率比较,差异具有统计学意义(P<0.05)。全市城区与郊区的社区卫生服务机构上转率比较,差异无统计学意义P>0.05);城区与郊区的综合医院下转率比较,差异亦无统计学意义P>0.05)。结论目前北京市社区卫生服务机构在双向转诊过程中处于相对单向流动状态,即社区卫生服务机构的上转率明显高于综合医院的下转率;城区与郊区间的地区差距不是造成这一结果的主要原因。
Objective To study the two-way referral status between community health service institutions and hospitals in Beijing, and to find out the existing issues and to analyze their reasons.Methods The person-times were collected from March to September 2008., including that visiting in 2 082 community health service institutions in 18 districts or counties in Beijing, that of upward-referral （from community health service institutions to general hospitals） and that of downward- referral （from general hospitals to the community health service institutions）. The upward-referral rate and downward-referral rate were calculated. Then the upward-referral rates and downward-referral rates between the urban and rural areas were compared respectively. Results The upward-referral rate in all the 18 districts or counties was significantly higher than downward-referral rate （P 〈 0.05）. There were no significant difference between the urban and the suburb in Beijing in both upward-referral rate and downward-referral rate （P〉 0.05）. Conclusion The upward-referral rare from community health service institutions to general hospitals is higher than the downward-referral rate from general hospitals to community health service institutions now in Beijing, being in a relatively single-directional flowing state. And the regional deference between urban and suburb in two-way referral is not a major cause of it.
