摘要
目的提高对螺旋CT诊断急性胰腺炎的认识。方法回顾性总结分析20例急性胰腺炎病例,包括临床表现、影像学检查。所有病人均采用西门子16排螺旋CT机扫描,上腹部平扫加增强扫描。结果全部患者均有腹痛,病程15h～4d。伴畏寒、发热者4例,10例有压痛,其中6例有反跳痛,4例有肌紧张,5例有黄疸,1例有浮肿,其中有2例做过B超检查考虑急性胰腺炎。影像学多表现为胰腺体积增大、胰周渗出性病灶、肾前筋膜增厚、假性囊肿等。结论螺旋CT对急性胰腺炎的诊断敏感性高、准确率高,绝大部分病例可以确诊,而且还可以判断炎症的范围及程度。图像直观,直接显示胰腺肿大程度、渗出范围、是否有坏死、出血,还能显示胰腺周围组织器官的受累情况,值得我们推广使用。
Objective To increase the spiral CT diagnosis of acute pancreatitis understanding. Methods Retrospectively analyzed 20 cases of acute pancreatitis cases,including clinical manifestations,imaging examination. All the patients were using Siemens 16-slice CT scan machine,on the abdomen scan and enhanced scanning. Results All patients had abdominal pain,the course from 15 to 4 hours. With chills, fever, four cases, 10 cases have tenderness, of which six cases of anti-Tiaotong, four cases have muscle tension, there are five cases of jaundice, one case of a swollen,including two cases did consider the B-examination of the pancreas Yan. Imaging showed more volume increased pancreas,pancreas weeks exudative lesions and kidney before the fascia thickening, pseudocyst, and so on. Conclusion The spiral CT in the diagnosis of acute pancreatitis high sensitivity,high accuracy rate,the vast majority of cases can be diagnosed,but also can determine the scope and extent of inflammation. Visual images, show that direct pancreatic enlargement of the scope of leakage ,whether necrosis, bleeding,but also revealed that the pancreas surrounding tissues and organs involved, we should promote the use of.
