摘要
目的分析法洛四联症（TOF）的病变特点，总结其手术治疗经验及术中所遇到的特殊情况。方法回顾性分析我院自2008年5月至2009年8月间实施的244例TOF手术情况，在低温、体外循环下行TOF根治术236例（包括合并畸形的手术），其中根治术同时行双向格林手术、室缺补片打孔术、右室至肺动脉带瓣血管外通道术共5例，介入＋根治（Hybrid）手术4例，非体外循环下体肺分流术8例。结果术后早期（30d内）死亡4例，手术死亡率1．64％。死亡原因为低心排综合征、心功能衰竭2例，灌注肺、呼吸衰竭2例。其他主要并发症30例。随访1-13个月，晚期死亡1例，死于右室－肺动脉外通道堵塞。结论准确掌握手术指征，根据不同病变采取不同术式是手术成功的关键。
Objective To analyze the pathologic characteristics of teralogy of Fallot （TOF）and summarize the experience of the operation for TOF. Methods 244 TOF cases having been performed operation in our hospital from May 2008 to August 2009 were retrospective analyzed. Among them one-stage 236 cases, two-stage 8 cases. Results 4 cases died in early stage after operation （〈30 d）, and the hospital mortality was 1.64%. Two of them died from severe low cardiac output syndrome, and two died from perfusion lung and respiratory failure. Follow up 1-13 months, 1 case died from arrhythmia caused by occlusion of valved artificial blood vessel in late stage. Conclusions Master the operation indication and perform with different methods in TOF patients with different pathologic features are the key points of successful operation.
