摘要
目的研究丹参注射剂对预防老年乳腺癌手术后皮瓣坏死的作用。方法98例临床Ⅰ～Ⅱ期65岁以上乳腺癌患者,随机分为治疗组(48例)与对照组(50例),术中用电刀分离薄皮瓣。手术后,对照组常规处理,治疗组用与对照组相同的常规处理并加注射用丹参400mg共5d,观察两组患者出现皮瓣坏死的例数及程度、住院时间、总体治疗费用。结果治疗组及对照组术后分别有9例、23例皮瓣坏死(P<0.01),且治疗组皮瓣坏死程度显著低于对照组(P<0.01)。治疗组住院(14.5±4.0)d,对照组(17.5±10.5)d(P<0.01)。治疗组术后再植皮3例,对照组10例(P>0.05)。治疗组总费用为(1600±460)元,对照为(2200±730)元(P<0.01)。治疗组放置引流管(6.0±3.5)d,对照组(6.0±3.0)d(P>0.05)。两组间不良反应无统计学差异。结论术后使用丹参注射剂能显著降低老年乳腺癌患者皮瓣坏死的发生率及程度,加快伤口愈合,减少住院天数及总体医疗费用。
Objective To investigate the effect of Danshen injection on preventing skin flap necrosis in senior patients after breast carcinectomy. Methods A total of 98 breast cancer patients （ 〉 65 year old, and classified as stage Ⅰ and stage Ⅱ ） were randomly divided into treatment group （48 cases） and control group （50 cases）. All the breast skin flaps were separated into thin flaps by electric knife in the process of operation. After the operation, patients in the control group receive conventional treatment, while patients in the treatment group received the same conventional treatment as well as 400 mg Danshen injection daily for 5 d. The extent and the number of skin flap necrosis, the hospitalization duration, and the overall cost of the treatment were studied between 2 groups. Results After operation, there were 9 cases and 23 cases of skin flap necrosis in the treatment group and the control group, respectively （ P 〈 0.01 ）, at the same time the extent of flap necrosis in patients of the treatment group was significantly milder than that of the control group （P 〈0. 01 ）. The hospitalization duration of patients in the treatment group was shorter than that of the control group[ （14.5 ±4.0） d vs （17.5 ±10.5 ） d, P 〈0.01 ]. The total cost of patients in the treatment group were significantly cheaper than that of the control group （1600 ± 460 vs 2200± 730, P 〈 0.01 ）. The number of the cases with skin grafts, the duration of remained drainage tube, and adverse effects were no significant difference between the 2 groups （P 〉 0.05）. Conclusion Using Danshen injection in senior patients after breast carcinectomy can significantly reduce the incidence and the extent of skin flap necrosis, speed up wound healing, and decrease hospitalization duration and medical costs.
