摘要
目的探讨高龄患者置入心脏起搏器的围手术期相关危险因素。方法选择置入心脏起搏器的患者232例,将年龄≥80岁的患者作为高龄组(78例),65～80岁的患者为老年组(98例),<65岁的患者为非老年组(56例),回顾分析患者围手术期临床事件,并进行分析。结果高龄组患者锁骨下静脉放置起搏器比例高于老年组和非老年组(P<0.01),手术时间长于老年组和非老年组(P<0.001);围手术期临床事件发生率高于老年组和非老年组(P=0.043,P=0.031),临床事件与年龄相关(r=8.722,P=0.016)。结论高龄患者置入心脏起搏器围手术期临床事件发生率高,应严格把握起搏器置入指征。
Objective To investigate the risk factors during perioperative period of implantation of the cardiac pacemaker for the first time in octogenarians with correlation analysis. Methods Analysis of risk factors was carried out to assess the clinical events during operation procedure of implantation of cardiac pacemaker and perioperative period in 78 octogenarians（group Ⅰ ）, compared with patients 〈80 years of age（group Ⅱ ） who received implantation of the permanent pacemaker through retrospective analysis. Results In group I, the rate of subclavian vein puncture （P 〈0.01） was higher,operation time （P 〈 0.001） was longer,incidences of operationrelated complications （P=0. 043） and operation-unrelated complications in perioperative period （P ： 0. 031） were higher than in group Ⅱ. Correlation analysis of these risk factors showed that clinical events in perioperative period was correlated to the agedness （r = 8. 722,P = 0. 016）. Conclusion The incidence of clinical events in perioperative period is high in octogenarians who received implantation of the cardiac pacemaker, and is related to the agedness.
