摘要
目的探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者血清心肌酶谱的变化及其意义。方法测定48例OSAHS患者和30例正常对照组的血清肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、天冬氨酸转氨酸(AST)、乳酸脱氢酶(LDH)及α-羟丁酸脱氢酶(-αHBDH)水平,并将血清心肌酶水平与睡眠呼吸暂停低通气指数(AHI)、平均最低动脉血氧饱和度(SaO2)进行直线相关分析。结果重度OSAHS患者组血清心肌酶水平均高于轻中度OSAHS组和正常对照组,CK(125.00±104.00)U/L vs(79.50±45.75)U/L和(77.50±27.00)U/L(均P<0.01);CK-MB(12.00±7.50)U/L vs(8.00±7.00)U/L和(7.00±4.25)U/L(均P<0.01);AST(28.00±14.00)U/L vs(18.00±7.25)U/L和(20.50±4.50)U/L(均P<0.01);LDH(156.50±36.75)U/L vs(128.50±36.00)U/L和(128.50±18.50)U/L(均P<0.01);-αHBDH(135.50±38.25)U/L vs(117.50±29.50)U/L和(102.50±24.00)U/L(均P<0.01),轻中度OSAHS组与正常对照组差异无统计学意义。相关分析显示CK、CK-MB、AST、LDH和-αHBDH水平与OSAHS患者的AHI呈正相关(P<0.01或<0.05),与平均最低SaO2呈负相关(P<0.01或<0.05)。结论OSAHS患者血清心肌酶有不同程度的升高,并随着病情的加重和缺氧的严重程度而升高。
Objective To investigate the serum levels of myocardial enzymes and its clinical significance in patients with ohstruetive sleep apnea-hypopnea syndrome(OSAHS). Methods The levels of creatine kinase(CK) and its isoenzyme ( CK-MB), aspartate aminotransferase ( AST), lactate dehydrogenase ( LDH ) and α-hydroxybutyrate dehydrogenase(a-HBDH) were measured in 48 patients with OSAHS and 30 healthy subjects. Furthermore, the correlation between the serum levels of myocardial enzymes and apnea hypopnea index(AHI) and the average lowest oxygen saturation of arterial blood(SaO2) was analyzed. Results The levels of myocardial enzymes in severe OSAHS patients were higher than those of mild moderate OSAHS patients and healthy controls,CK(125. 00±104.00) U/L vs (79.50±45.75)U/Land (77.50±27.00) U/L(allP〈0.01); CK-MB(12.00±7.50) U/L vs (8. 00± 7.00) and (7.00±4.25) U/L(all P 〈0.01) ; AST(28.00±14.00) U/L vs (18. 00±7.25) U/L and (20.50±4.50)(all P 0.01) LDH(156.50±36.75) U/L vs (128.50±36.00) and (128.50± 18.50) U/L(all P 〈0.01 ); α-HBDH (135.50 ± 38.25) U/L vs (117.50 ±29.50) and (102.50 ±24.00) U/L(all P 〈0.01). The levels in mild-moderate OSAHS patients and in healthy controls had no significant difference. The levels of serum myocardial enzymes in OSAHS patients were correlated positively with AHI, but were correlated negatively with the the average lowest SaO2 ( P 〈0.01 or 〈0.05). Conclusion The serum levels of myocardial enzymes are more or less higher in OSAHS patients, increasing with the degree of severity and hypoxemia of OSAHS.
