MRA在脑梗死患者颅内外动脉病变分析中的应用 被引量:5

Application of magnetic resonance angiography in the analysis of intracranial and extracranial arterial diseases in cerebral ischemic patients

摘要 目的采用磁共振血管成像(magnetic resonance angiography,MRA)对脑梗死患者颅内外动脉病变进行分析。方法以125例脑梗死患者为研究对象,详细询问危险因素,行心电图、心脏超声、头颅CT/MRI、颈部血管及头颅血管MRA检查,了解动脉病变部位、特点,对颅内外动脉病变与危险因素的相关性进行探讨。结果125例患者中颅内动脉病变54例(43.2%),颅外动脉病变64例(51.2%),其中颅内外动脉均有病变者17例(13.6%),未见明显颅内外动脉病变者24例(19.2%);颅内动脉病变与各危险因素的相关性依次为高血压(66.7%)、吸烟(35.2%)及高脂血症(25.9%),颅外动脉病变与各危险因素的相关性依次为高脂血症(48.4%)、糖尿病(39.1%)及吸烟(29.7%)。结论中国人脑梗死患者颅内动脉和颅外动脉硬化性病变的比例均近50%。颅内动脉病变主要与高血压和吸烟有关,颅外动脉病变主要与高血脂和糖尿病有关。颅内外动脉同时存在病变者不高,仅占13.6%。 Objective To analyze the intracranial and extracranial arterial diseases in cerebral ischemic patients by using magnetic resonance angiography (MRA). Methods A total of 125 cerebral ischemic patients were enrolled in this study, and MRA was performed in all the patients to detect the focus and characteristics of the arterial diseases. Meanwhile, the risk factors were questioned, and the results of ECG, UCG and cranial CT/MRI scans were collected. Results Among all the 125 cerebral ischemic patients, there were 54 patients (43.2%) with intracranial arterial disease, 64 patients (51.2%) with extracranial arterial disease, 17 patients (13.6%) with both intrcranial and extracranial arterial diseases, and 24 patients (19.2%) with no arterial disease. The risk factors related to intracranial arterial disease were hypertension (66.7%), smoking (35.2%) and hyperlipemia (25.9%), whereas the risk factors related to extracranial arterial disease were hyperlipemia (48.4%) , diabetes (39. 1% ) and smoking (29.7%). Conclusion Either intracranial or extracranial arterial disease accounts for a high proportion in Chinese cerebral ischemic patients, about 50% each. Intracranial arterial disease mostly relates to hypertension or smoking, while extracranial arterial disease mostly relates to hyperlipemia or diabetes. The proportion of Chinese cerebral ischemic patients with both intrcranial and extracranial arterial diseases is modest, only 13.6%.
