摘要
目的探讨人工肱骨头置换术治疗老年骨质疏松性肱骨近端粉碎性骨折的疗效。方法对26例NeerⅢ-Ⅳ型老年骨质疏松性肱骨近端骨折行人工肱骨头置换术。男12例,女14例;年龄64～86岁,平均73.3岁。左侧10例,右侧16例。Neer分型Ⅲ型8例,Ⅳ型18例。结果术后随访12～36个月,按Neer评分系统,26例中优8例,良14例,可3例,差1例,优良率84.6%。结论人工肱骨头置换术治疗老年肱骨近端粉碎性骨折近期疗效满意,重视肩袖损伤修复及术后早期肩关节功能锻炼是提高疗效的关键。
Objective To investigate the effect of humeral head prosthesis replacement for osteoporotic fractures in the proximal humerus in aged patients . Methods Twenty-six aged patients with osteoporotic fractures in the proximal humerus underwent humeral head prosthesis replacement. There were 12 men and 14 women, aged from 64 to 86 with the average age of 73.3. The replacement was done on the left side（n=10） and the right side（n=16）. According to Neer classification, the fracture was type Ⅲ（n=8） and type Ⅳ（n=18）. Results All patients were followed up for 12 to 36 months. According to Neer score, the result was excellent in 8 patients, good in 14, ok in 3 and poor in 1. The excellent rate was 84.6%. Conclusions Humeral head prosthesis replacement shows satisfactory short-term results for osteoporotic fractures in the proximal humerus in aged patients. A solid reconstruction for rotary cuff of the shoulder and early rehabilitation after operation were important to improving the postoperative results.
