摘要
目的:探讨缺血性脑血管病患者血清超敏C-反应蛋白(hs-CRP)水平,与MR弥散成像(DWI)及缺血损伤的严重程度的关系,从而判断血清hs-CRP水平在急性缺血性脑血管病早期诊断及预后评估中的意义。方法:收集2008年1-7月在我院神经内科住院的缺血性脑血管病患者182例,记录其影像学(DWI)资料、入院后2 h内血清hs-CRP水平。分成3组,即急性脑梗塞组、非急性期脑梗塞组及短暂性脑缺血发作组,并选择健康体检者33例作为对照组,急性脑梗塞组按照DWI上显示的病变部位及面积分为3个亚组(A组:TIA组;B组:小面积梗塞组,C组:大面积梗塞组)。比较各组间血清hs-CRP水平,并与DWI进行相关分析。结果:血清hs-CRP水平在急性脑梗塞组最高,TIA组次之,差异具有统计学意义(P<0.01);非急性期脑梗塞组与正常对照组间差异无显著性(P>0.05)。Spearman’s相关分析显示hs-CRP水平与DWI显示的梗死部位及面积有显著相关性(P<0.01)。结论:hs-CRP水平在急性缺血性脑血管病患者中升高,与缺血的范围及严重程度密切相关;与DWI显示的梗死面积及部位具有相关性,两者结合可共同用于脑缺血的早期诊断和病情严重程度的评估。
Objective: To determine the diagnostic value of the serum high sensitive C-reactive protein(hs-CRP) level in ischemic cerebrovascular disease and to compare the level of hs-CRP with the diffusion-weighted imaging(DWI) results in acute stroke.Methods: One hundred and eighty-two patients with ischemic cerebrovascular disease were divided into three groups: acute cerebral infarction,transient ischemic attack(TIA),and non-acute cerebral infarction groups,and another thirty-three healthy subjects were involved in this study as control group.The level of serum hs-CRP in different groups was determined and compared and the correlation to DWI results was analyzed.Results: The serum hs-CRP in acute cerebral infarction group were significantly higher than in other groups(P〈0.01).The serum hs-CRP in TIA group were significantly higher than that in non-acute cerebral infarction group and control group respectively(both P〈0.05),and there was no significant difference between non-acute cerebra...更多l infarction group and control group.Spearman rank correlation test demonstrated that there was a significant correlation between the serum hs-CRP level and the results of DWI(P〈0.01).Conclusion: The serum hs-CRP level of acute cerebral infarction and TIA group was increased,and was associated with the degree and extent of ischemic damage showed in DWI.Both serum hs-CRP assay and DWI could be used for early diagnosis and clinical assessment of ischemic cerebrovascular disease.
