Ultrasonic Features of Placental Premature Aging

摘要 目的:了解胎盘提前老化的超声征象。方法:回顾性分析33例妊娠37周超声观察到Ⅲ级成熟度的单胎妊娠产妇,并随机抽取同期60例足月后观察到Ⅲ级胎盘成熟度的单胎妊娠作对照,比较两组的孕龄、新生儿体重、羊水过少发生率和剖宫产率。结果:胎盘提前老化组的孕龄为(36.34±2.53)周,新生儿出生体重为(2 816.13±501.78)g,显著小于对照组(39.63±2.41)周和(3 441.26±423.17)g,P<0.001;羊水过少发生率(24.24%)和择期剖宫产率(87.88%)显著高于对照组(6.67%和46.63%,P<0.05)。结论:胎盘提前老化的超声征象、羊水过少和新生儿出生体重减少,可提示胎盘功能减退,应密切监护。 Objective: To analyze the ultrasonic features of placental premature aging. Methods: The perinatal outcomes and uhrasonic results of 33 pregnant women with preterm grade Ⅲ placenta and within 37 weeks of gestation were analyzed retrospectively, and were compared to those of 60 pregnant women with preterm grade Ⅲ placenta at full term (〉37 weeks). Results: The gesta- tional weeks and newborn weight were significantly lower than those in control group respectively (36.34±2.53) weeks versus (39.63±2.41) weeks, and (2 816.13±501.78) g versus (3 441.26 ±423.17) g, respectively, and both P〈0. 001,and the incidence of oligohydramnios (24. 24%) and rate of cesarean delivery (87.88 %) were significantly higher than those in the control group respectively (6.25% and 47.5%, both P〈0.05). Conclusion: The ultrasonic features of preterm grade Ⅲ placenta less than 37 weeks of gestation are correlated with oligohydramnois and low birth weight, and close monitoring of uhrasonography might help to predict placental dysfunction.
