听性稳态反应对噪声性聋客观评估的价值 被引量:2

The Application of the Auditory Steady-state Response to Patients with Noise Induced Hearing Loss

摘要 目的研究噪声性聋患者的听性稳态反应(ASSR)阈与纯音听阈的关系,为客观评估噪声性聋及伪聋提供依据。方法对40例(80耳)噪声性聋患者及20例(40耳)正常人分别进行ASSR与纯音听阈测试,分别比较0.25、0.5、1、2、3、4、6kHz各频率ASSR反应阈与纯音听阈的差值及相关性。结果正常组在0.25、0.5kHz的ASSR反应阈与纯音听阈相比差异无统计学意义(P>0.05),在1、2、3、4、6kHz的ASSR反应阈高于纯音听阈,二者相比差异有统计学意义(P<0.05)。噪声性聋组除2kHz外其余各频率ASSR的反应阈与纯音听阈相比差异无统计学意义(P>0.05),其ASSR反应阈与纯音听阈在0.25、0.5、1、2、3、4、6kHz处的相关系数分别为0.55、0.62、0.55、0.49、0.54、0.73、0.73。结论噪声性聋患者ASSR反应阈与纯音听阈显著相关,且随着听力损失的加重,ASSR反应阈越接近纯音听阈;ASSR可作为噪声性聋及伪聋患者的客观听力评估方法之一。 Objective To study the relations between auditory steady-state response (ASSR) and pure tone audiometry to assess the hearing status of patients with noise induced hearing loss and to provide evidence for the identification of nonfunctional hearing loss. Methods 80 ears (40 patients) with noise induced hearing impairment were tested with ASSR and pure tone audiometry and the results at 0.25, 0. 5,1,2,3,4, and 6 kHz were analyzed between the thresholds of ASSR and pure tone audiometry. Results The results of the group with normal hearing showed no statistical significance at 0.25, and 0.1 kHz between ASSR and pure tone audiometry(P〉0.05) while significantly different results at 1, 2, 3, 4, and 6 kHz were found between the two tests (P〈0.05). For the group with noise induced hearing impairment there were no statistical significances except 2 kHz between the two tests. The correlation coefficients of the thresholds at 0.25, 0.5, 1, 2, 3, 4, 6 kHz were 0.55, 0.62, 0.55, 0.49, 0.54, 0.73, and 0.73, respectively. Conclusion There are no statistically significant differences at 0.25, and 0.1 kHz between ASSR and pure tone audiometry for the group with normal hearing. ASSR has higher responses than that of pure tone audiometry. The thresholds of ASSR are positively correlated with that of pure tone audiometry. The correlation becomes closer between the two tests as hearing loss increases for the patients with noise induced hearing impairment. ASSR can be applied to the evaluation for the patients with noise induced hearing impairment.
