硝普钠、ATP控制性降压对全身血流动力学、冠脉循环及心肌代谢的影响 被引量:1

摘要 控制性低血压多用于心血管手术及心血管病的治疗,而此类病人维持良好的心肌灌注和氧供需平衡是维护心肌功能最重要的条件。各种降压药在不同的降压幅度下对心肌血流和代谢产生不同程度的影响。硝普钠、ATP均为直接扩血管药,对它们的降压特点及血流动力学效应国内外已有较多研究,但两药降压对心肌血流和代谢影响如何?国内尚未见报道,本文着重比较它们对麻醉犬心肌血流和代谢的影响。 Effects of hypotension induced dy Sodium Nitroprusside (SNP), Adenosine Triphosph ate(ATP)on systemic(?)anb coronary hemodynamics anb myocardial metadolism were studied in 12 dogs anesthetized with 1% procain fentanyl balanced anesthesia.Either 0.01% SNP or 0.5% ATP was given to reduce mean blood pressure (MAP) by 50% for one hour.No. significant change of cardiac index anb stroke volume index was observed in both groups Heart rate decreased (P<0.05) after ATP administration and maintained at previous level in SNP group.ATP-induced hypotension resulted in a significant increase in myocardial blood flow (MBF) and myocardial O_2 supply, myocardial O_2consumption maintainedor decreased. Incontrast, SNP induced hypotension did not significantly increase MBF and myocard ial O_2 supply,but MVO_2 and myocardial O_2 extraction remained unchanged.The results indicate that the balance between myocardial O_2 supply and demand was finely maintained in both groups.
