摘要
目的探讨隐神经营养血管蒂逆行岛状皮瓣修复下肢软组织缺损的临床效果。方法对9例小腿下1/3踝关节周围皮肤坏死及缺损的患者,行隐神经营养血管蒂逆行岛状皮瓣转移修复术,皮瓣面积6cm×5cm～12cm×8cm,平均9cm×6.5cm。结果8例全部成活一期愈合,1例皮瓣部分皮肤坏死经换药愈合。随诊时间6个月～2年,平均11个月,术后足内侧皮肤感觉早期减退,6个月恢复,皮肤质地优良、耐磨,外观良好。结论隐神经营养血管蒂逆行岛状皮瓣修复下肢软组织缺损取材方便,血供可靠,不牺牲主要动脉。
Objective To investigate the clinical efficacy of reversed saphenous neurocutaneons island flap on the repair of soft tissue defect in lower limbs. Methods The repair therapy of reversed saphenous neurocutaneous island flap was applied to 9 cases of peripheral skin necrosis and defect in ankle joint 1/ 3 inferior to the leg. The flag area ranged from 6cm×5cm～12cm×8cm ,averagely 9 cm×6.5 cm. Resuits All of the flaps were survived in 8 case and 1 case encountered partial necrosis,but cured after dressing. The follow - up duration ranged from 6 months to 2 years, averagely 11 months. After operation, the skin sensation reduced at early stage on the medial side of foot, but recovered in 6 months. The skin was excellent in quality,high in abrasion and good in appearance. Conclusion Reversed saphenous neurocutaneous island flap on the repair of soft tissue defect in lower limbs is simple in material collection and reliable in blood supply without the injury of main arteries.
