摘要
目的研究危重病人在肠内营养期间误吸发生率降低的护理策略,减少吸入性肺炎的发生。方法统计2007年危重病人进行肠内营养期间发生误吸的情况,分析引起误吸的各种原因,制定相关的护理指引,由护理组长督促落实临床执行情况并评价护理效果,并对2008年危重病人肠内营养期间预防误吸发生的护理方法进行前瞻性研究,两组误吸发生率比较进行χ2检验。结果针对发生误吸的原因,制定了切实可行的肠内营养护理指引,并由高级责任护士严抓落实,病人出现误吸的现象明显减少。结论危重病人肠内营养的安全管理,护理人员不但需要有丰富的专业知识、护理安全隐患意识,懂得正确、适时的评估,给予恰当、有效的预防干预,护理管理方面还需要人力资源设置及层级管理原则,抓环节质量管理,这样才能杜绝安全隐患。
Objective To study the nursing strategies for reducing the incidence of aspiration for critical patients during enteral nutrition, and to reduce the incidence of aspiration pneumonia. Method The incidence of aspiration for critical patients during enteral nutrition was collected and analysis during 2007, the cause of aspiration was analyzed for set up nursing instruction. The clinical carry out and effect evaluation of these measurement was conducted by nursing supervisor, and conduct the prospective nursing study for aspiration preventing for critical patients during enteral nutrition period in 2008. the incidence rate of aspiration between the two groups was compared analyzed by x2 test. Result Based on the cause of aspiration, the practicable nursing instruction of enteral nutrition for critical patients was set up and carried out by high duty nurse. The incidence of aspiration among patients was remarkable reduced. Conclusion For better enteral nutrition safety management of critical, the nurses needs plentiful professional knowledge, sense of nursing safety, correct and timing evaluation, suitable and effective intervention. Allocation of nursing manpower, grading management and quality control was also necessary for nursing management and eliminating potential safety problems.
