摘要
目的了解在婚男男性行为人群(Men who have sex with men,MSM)HIV和梅毒感染率及其影响因素,为开展有针对性的干预工作提供建议。方法通过滚雪球的方法招募被调查对象,由经统一培训的调查员进行面对面的问卷调查,采集血样检测HIV、梅毒和丙肝抗体。数据经双录入核对后采用SPSS12.0进行统计分析。结果在调查的192名在婚MSM中,艾滋病防治知识晓率为80.7%,82.8%发生首次性行为的性伴是女性,最近一次与最近六个月同男性发生肛交时安全套使用率分别为53.8%和23.1%。最近一次及最近六个月与女性发生性行为时安全套使用率分别为25.0%和18.3%。HIV、梅毒和丙肝感染率分别为26.0%、9.4%和7.3%。结论在婚MSM人群HIV感染率高,与男性及女性性伴发生性行为时安全套使用率低,他们作为桥梁人群将促进艾滋病由MSM人群向一般人群传播,开展针对该亚人群的艾滋病防治工作是MSM人群艾滋病防治的重点。
Objective To survey the prevalence of HIV and syphilis and the influential factors among married MSM . Methods MSM recruited by snowball were surveyed by the investigator. Blood samples were taken to detect antibodies to HIV,syphilis and hepatitis C. Data were entered double and analyzed by Software SPSS12.0. Results The HIV/AIDS knowledge of 192 married MSM was 80.7% and 82.8% of them had first sex with the female partner. The rate of condom use during anal sex with men in the last time and in the past six months was 53.8% and 23.1% respectively. And the rate of condom use during sex with women in the last time and in the past six months was 25.0% and 18.3% respectively. The prevalence of HIV,syphilis and hepatitis C was 26.0%,9.4% and 7.3% respectively. Condom use during sex with women in the last time（Condom use VS no condom use： AOR=2.9,95%CI=1.0-8.5 ） was the independent factor of HIV infection, senior MSM （age group of 50 years old and above VS age group of 20 years old to 29 years old： AOR=10.7,95%CI=1.2-92.8）, young age of first sex with men （age group of younger than 18 years old VS age group of above 25 years old： AOR=8.2,95% CI=1.5-43.4） and HIV infection （AOR=3.4,95%CI=1.1-10.4） was the independent factor of Syphilis infection. Conclusion high HIV prevalence,low rate of condom use during anal sex with men and sex with women among married MSM and its bridge role of HIV transmission will accelerate HIV spread from MSM to general population. It is crucial to conduct the intervention against this subgroup MSM for AIDS campaign among MSM.
