多层螺旋CT冠状动脉成像技术在冠心病診断中的临床应用

摘要 目的:探讨多层CT冠状动脉成像(CTCA)的临床应用价值。材料与方法:对52例患者(临床疑诊为冠心病者49例,支架置入术后3例)进行回顾性心电门控平扫及增强,后处理用Vessel View软件包对所有病人做了多平面重建(MPR)、最大密度投影法重建(MIP)、容积再现技术(VRT)重建及血管剖面技术分析;其中30例可疑冠心病患者有选择性冠状动脉造影(SCA)作比较。结果:83.3%的冠脉节段图像质量可以用于评价,可较准确地显示冠脉通畅情况及管壁病变。与SCA比,CTCA敏感度和特异度分别为85.1%和92.3%。结论:16层CTCA可显示较长节段的冠状动脉,较准确检測冠脉病变,有较高的临床应用价值。 Purpose: To evaluate the main clinical value of 16 - slice spiral CT coronary anglography(CTCA). Mate. rials and Methods: Plain and enhanced imaging were performed by retrospective ECG gating in 52 patients, including 49 eases suspected coronary artery disease and 3 eases of coronary stent. Post - processing was conducted with Vessel View soft package, in whleh MPR, M/P, VRT and section plane of coronary artery were made in all eases, among them 30 eases underwent selective coronary angiography(SCA)as a eomparision. Results: 83.3% of the coronary segments in CTCA could aeeulately display the pateney of the coronary arteries and the pathological changes in vessel walls. In comparisan with SCA, the sensitivity and specificity were 85. 1% and 92. 3% respectively. Conclusion: 16 - slice CTCA can display long segment of coronary artery and assess coronary artery disease with high aeeuraey, which has highly clinical value.
