摘要
目的:探讨布地奈德联合沙丁胺醇雾化吸入对AECOPD的疗效及安全性。方法:将54例AECOPD患者随机分为两组,治疗组27例,对照组27例,在常规治疗的基础上治疗组给予布地奈德联合沙丁胺醇雾化吸入,对照组单用布地奈德雾化吸入,观察治疗前、治疗3d、治疗7d后患者咳、喘、气短、胸闷等临床症状的评分、FEV1占预计值的比值、血生化、生命体征等指标的变化及有无不良反应出现。结果:治疗组及对照组在治疗3d后FEV1占预计值的比值,各临床症状的评分均明显改善(P<0.01,P<0.05),但两组间比较治疗组优势更明显(P<0.05)。治疗7d后两组各项指标进一步改善,但两组间差异不明显(P>0.05)。观察期内无不良反应,各项生化及生命指标均正常。结论:布地奈德联合沙丁胺醇雾化吸入在AECOPD治疗早期较单一吸入布地奈德临床症状缓解更加迅速,肺功能改善明显,观察期内无不良反应,值得临床推广。
Objective： To evaluate the curative effect and safety of nebulization inhaling of budesonide and salbutamol in acute exacerbations of chronic obstructive pulmonary disease （AECOPD）.Methods：54 suffers with AECOPD were randomly divided into two groups（treatment group 27 cases,control group 27 cases）.On the base of rotine therapy,the patients in treatment group were treated with nebulization inhaling budesonide and salbutmol while another patients in control group were treated only with inhaling budesonide.The score of clinical symptoms of suffers＇ cough,asthma,shortness of breath and thorax suffocation were observed before therapy,after 3-day and 7-day therapy respectively.In addition,the index changes of the forced expired volume in one second （FEV1%）, blood biochemistry and vital physical symptoms,and adverse reactions were observed.Results：Pulmonary function （FEV1%）and the score of every clinical symptom were improved in evidence （P 〈 0.01 ;P 〈 0.05）in the two groups after 3-day therapy.However,with the comparison of the two groups,the advantage of the treatment group was more distint（P 〈 0.05）.Every index of the two groups was further improved after 7-day therapy,but the discrepancy was not distinct between the two groups（P 〉 0.05）.There was no adverse reaction within the period of observation. Every blood biochemistry and physical symptom was normal.Conclusion：In the forepart of AECOPD therapy,nebulizution in haling of badesonide associated with salbutamol was quicker than that of budesonide singly in the clinic symptom anesis.Pulmonary function was improved distinctly.There was no adverse reaction with period of observation.It is worthy of popularizing.
