摘要
目的:针对骶髂关节部位骨折脱位,对骶髂关节周围神经的分布进行解剖学研究,了解神经损伤机制,确定钢板及螺钉的最佳位置。方法:对20具防腐成年尸体骨盆标本,共40侧依次选用不同的平面,从前方入路充分显露骶髂关节及周围神经,观察神经走行特点并测量神经到骶髂关节的水平距离。结果:在L5神经前支出椎间孔处,与骶髂关节的距离为(2.44±0.19)cm,在L5神经前支出椎间孔平面,L4神经前支与骶髂关节的距离为(1.85±0.04)cm,腰骶干在其形成汇合点处、平骶岬处、在骨盆环处距骶髂关节的距离分别为(1.33±0.05)cm,(1.45±0.05)cm,(1.05±0.04)cm。在20具尸体标本上测量发现L4,L5神经根前支及腰骶干距骶髂关节距离最短处位于骨盆环。结论:随L4,L5神经前支及腰骶干逐渐向外下方走行,L4,L5神经前支和腰骶干的外缘与骶髂关节的距离在不同平面逐渐变小,在骨盆环处腰骶干与骶髂关节距离最小。
Objective: Directing against the fracture and dislocation of sacroiliac joint,to study its anatomy of the nerves,and the mechanisms of the nerves injuries,for providing the instruction for the internal fixation,and the best position of plate and screw.Methods: The study was performed in 20 emblamed pelvis specimens,exposed the sacroiliac joint and the nerves,the location of the nerves were observed and the distances between the nerves and sacroiliac joint were measured. Results: The distance between the anteri-or branches of lumber nerve 5 and the sacroiliac joint as it exits from the interwertebral foramen; the distance between the anterior branches nerve 4 and sacroiliac joint at the level where the lumber nerve 5 exits the intervertebral foramen,the distance between the beginning of the lumbosaeral trunk(LST) and the sacroiliac joint,the proximity of the LST to the sacroiliac joint at the promontorium and the proximity of the LST to the sacroiliac joint at the pelvic brim were as follows : (2.44 ± 0.19 ), ( 1.85 ± 0.04), ( 1.33 ± 0.05 ), ( 1.45 ± 0. 05 ), ( 1.05 ± 0.04 ) cm. Conclusion : To follow anterior branches of IA, LS, LST migrating lateral and inferior,distance between the nerve and the joint became shorter at different levels. The shortest distance between the nerve and the joint is the one between the LST and the sacroiliac joint at the pelvic brim.
