摘要
ＴＴＰ是一种少见的，包括脑在内的全身脏器微动脉和毛细血管广泛闭塞的严重疾病。短暂的局灶性神经系统症状相当常见，并常常为首发症状。本文报告一例已妊娠２５周的病例，因突发左侧无力入院，体检为左侧轻瘫。实验室检查包括微血管病性溶血性贫血，抗人球蛋白试验阴性，血小板减少。ＣＴ显示右大脑中动脉皮质支分布的颞顶大片低密度灶。病人发病后轻瘫持续存在，八个月后因左侧无力加剧再次入院，ＣＴ提示原梗塞灶扩大。病人碓诊为ＴＴＰ后使用血浆输入治疗，病情迅速改善。偶尔，我们也可以见到ＣＴ证实为较大梗塞的，表现为持续轻偏瘫的ＴＴＰ病人。
TTP is an uncommon but serious disease of wide spread occlusions of arteriols and capillaries involving practically all organs of the body, including the brain.Transient focal neurological symptoms are practically present and are often the initial clinical manifestations of the disease.We report a case of 28 year old woman who entered the hospital with rapid left limb weakness.Again she was in the 25th week of pregnancy.Physical exam revealed she had left paresis.Laboratory studies indicated a coombs' negative miroangiopathic hemolytic anemia and thrombocytopenia.CT showed a gross hypodensity lesion in right temporo pariental lobe attributed to major cortical branch occlusion of middle cerebral artery.She remained hemiplegic 8 months later when she had a relapse and entered the hospital again with increased left limb weakness.CT showed the enlarged volume of previous infarction.Diagnosed as TTP she was treated with plasma infusion and got a dramatic improvement.The persistent focal neurological deficit with gross CT documented infarction could occasionally be found in patients with TTP.
