摘要
目的探讨彩色多普勒超声在第一跖背动脉分型中的意义。方法对50例足趾移植患者的第一跖背动脉行彩色多普勒血流显像(CDFI),根据超声血管影像按其解剖位置、血管内径及峰值血流速度进行分型,并与手术结果进行对照。结果第一跖背动脉按解剖位置分为:①浅表型:9例占18.00%;②肌内型:28例占56.00%;③肌下型:13例占26.00%。按内径分:①大型:内径>1.5mm,共9例占18.00%;②中型:内径在1.0~1.5mm之间,共32例占64.00%;③小型:内径<1.0mm,共9例占18.00%。按血流速度分型:①快速血流型:血流速度>20cm/s,共16例占32.00%;②中速血流型:血流速度位于10~20cm/s之间,共30例占60.00%;③低速血流型:血流速度<10cm/s,共4例占8.00%。CDFI分型与术中所见完全吻合。结论彩色多普勒超声用于第一跖背动脉血管分型准确、可靠,对临床手术方案的制定具有重要意义。
Objective To assess the significance of color Doppler ultrasonography in typing of the first dorsal metatarsal artery.Methods A total of 50 patients who received toe transplantation underwent color Doppler flow imaging(CDFI) to examine the first dorsal metatarsal artery.Then the first dorsal metatarsal artery was typed according to anatomical position,internal diameter and peak velocity.The results were compared with surgical outcomes.Results ①According to the anatomy position,the first dorsal metatarsal artery was typed into superficial type(n=9,18.00%),intramuscular type(n=28,56.00%) and inframuscular type(n=13,26.00%).②According to the internal diameter,the first dorsal metatarsal artery was typed into large(〉1.5 mm),medium(1.0—1.5 mm) and small(〈1.0 mm) type,with patients of 9(18.00%),32(64.00%) and 9(18.00%),respectively.③According to the peak velocity,the first dorsal metatarsal artery was typed into fast(〉20 cm/s),medium(10—20 cm/s),and low(〈10 cm/s) type,with patients of 16(32.00%),30(60.00%) and 4(8.00%),respectively.CDFI typing of the first dorsal metatarsal artery was identical to the surgical results.Conclusion CDFI can be used for the typing of the first dorsal metatarsal artery with accuracy,having important significance to the design of clinical surgery program.
