2型糖尿病患者糖化血红蛋白与餐后血脂谱的关系

Relationship between glycohemoglobin and postprandial serum lipid profile in patients with type 2 diabetes

摘要 目的探讨不同糖代谢控制状态下2型糖尿病患者餐后血脂谱的差异。方法按照GHbA1c水平将120例2型糖尿病患者分为3组:A组,GHbA1c〈6.5%(n=48);B组,6.5%≤GHbA1c〈7.5%(n=34);C组,GHbA1c≥7.5%(n=38)。比较3组患者餐后血脂谱的差异,包括三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL—C)、低密度脂蛋白胆同醇(LDL—C)、TG/HDL—C及LDL—C/HDL—C。逐步回归分析法探讨GHbA1c与患者一般情况及餐后血脂谱的关系。结果C组餐后TG水平高于A组[1.79(0.58~9.69)mmol/L比1.71(0.54~4.66)mmol/L,P〈0.05],而HDL—C水平低于A组[(1.12±0.28)mmol/L比(1.27±0.33)mmol/L,P〈0.05]。3组间餐后TC及LDL-C水平差异无统计学意义(均P〉0.05)。逐步回归分析显示,除餐后血糖与病程外,HDL—C(β'=-0.225,P=0.005)及TG与HDL—C的比值(β'=-0.334,P=0.023)与GHbA1c关系密切。结论2型糖尿病患者餐后HDL—C及TG水平可能影响糖代谢控制情况。 Objective To explore the difference of postprandial serum lipid profile in patients with type 2 diabetes in different control states of glucose metabolism. Methods One hundred and twenty patients with type 2 diabetes were divided into three groups according to the glycosylated hemoglobin Alc (GHbA1c) levels: GHbAIe〈 6.5% in group A (n=48), 6.5% ≤GHbA1c〈7.5% in group B (n=34) , GHbA1c≥7.5% in group C (n=38). The postprandial serum lipid profiles, including triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol(HDL-C), low density lipoprotein eholesterol(LDL-C), TG/HDL-C and LDL- C/HDL- C, in 3 groups were compared. Stepwise regression analysis was used to examine the correlations between GHbA1c and general conditions, postprandial serum lipid profile of patients. Results The level of postprandial serum TG was higher in group C than that in group A [ 1.79 (0.58±0.69) mmol/L vs 1.71 (0.54±4.66) mmol/L, P〈0.05]. The level of HDL-C was lower in group C than that in group A [ ( 1.12± 0.28) mmol/L vs (1.27±0.33) mmol/L, P〈0.05]. There was no significant difference in the levels of postprandial serum LDL-C and TC among 3 groups (all P〉0.05). Besides the postprandial plasma glucose and the duration of disease, postprandial sermn HDL-C (β' =-0.225, P=0.005 ) and TG/HDL-C (β' =-0.334, P=0.023) were correlated with GHbA1c by stepwise regression analysis. Conclusion The levels of postprandial HDL-C and TG may play a role in the glycometabolie control in the patients with type 2 diabetes.
