急性肾衰时血小板功能的研究

摘要 本实验通过对血小板功能指标的观察,发现家兔急性肾功能衰竭时,血小板粘附,聚集、释放功能都有显著增强;钙拮抗剂异搏停对血小板功能有抑制作用,并可以改善肾功能。这提示血小板在急性肾功能衰竭的发生中可能具有较为重要的作用。 The changes of platelet function in multiple system organ failure have been previously described by other authors. In the present study, we investigated the platelet function profiles in rabbits with acute renal failure induced by infusion of glycerol.76 rabbits were tested in this study, which were divided into 3 groups: normal control(n=12), glycerol group alone(n=30) and verapamil group(n = 34). Several parameters of platelet function as well as renal micropathologic changes by electron microscopy were studied at 3,6,24,48 and 72 hours following infusion of glycerol. The baseline (from normal control) Platelet adhesion, platelet aggregation, platelet count and platelet factor time were 28±21.4%, 15.8± 9.73%, 22.3±6.8×10~6mm^3 and 32. 6± 9.1 sec. respectively. In the rabbits with acute renal failure, the platelet count went markedly up and was 76% above that of the normal control at 48 hours following the infusion of glycerol, the platelet aggregation remained greater significantly and was about 3.5 times that of the normal control, the platelet adhesion rose and went to a peak, 52.8± 11.7%, at 6 hours after the infusion (P<0.01), and the platelet factor time was shorter than that of the normal control. As compared with the glycerol group alone, the platelet aggregation and adhesion were significantly decreased and the platelet factor time was longer in the verapamil group. In conlusion ,the platelet function was enhanced by infusion of glycerol, and was decreased in the verapamil group, compared with the glycerol group. It was pointing to a possible role of the platelet in the pathogenesis of glycerol-induced acute renal failure in rabbits ,and the platelet function inhibited by verapamil may be one of the mechanisms of verapamil protecting against acute renal failure.
