机械通气早产儿与足月儿疾病转归影响因素的研究

摘要 目的:探讨危重新生儿的机械通气治疗,分析围生期因素和相关干预措施等对疾病转归的影响。方法:回顾性分析淮安市妇幼保健院2007年1月至2008年12月有完整资料且以SIMV模式机械通气的86例早产儿与足月儿临床资料,按疾病转归分为治愈组和未愈组。比较早产儿与足月儿疾病转归影响因素的差异。结果:胎龄、出生体重、宫内窘迫、Apgar1分钟评分、呼吸机参数最高PIP及合并呼吸机相关性肺炎、固尔苏应用等是影响早产儿治愈率的重要因素;呼吸机参数初调及最高RR、颅内出血和惊厥等是影响足月儿治愈率的重要因素;静脉营养天数和沐舒坦的应用对早产儿和足月儿的治愈率均有显著的影响。结论:加强围生期高危因素的预防,及时防治并发症,合理使用呼吸机,同时辅以应用静脉营养等新技术可提高危重患儿的存活率和生存质量。
