跨血脑屏障药物转运的研究进展 被引量:19

摘要 血脑屏障(Blood-brain barrier,BBB)的存在成为人们治疗中枢神经系统疾病(Central nervous system,CNS)所面临的一道难题,因为基本上100%的大分子药物及大于98%的小分子药物均无法穿过血脑屏障.因此,如何使CNS药物跨越血脑屏障从血液进入脑内且发挥药效成为解决难题的关键所在.如今一些借助内源性BBB运载体使药物转运入脑的技术发展起来,并处于实验研究和临床试验阶段,例如借助载体介导的转运系统、受体介导的转运系统的药物治疗策略,以及纳米技术的运用等,都有着良好的应用前景.这些新发现及新技术将为跨血脑屏障药物转运的研究提供新思路,并有望实现对CNS疾病患者的成功治疗. Blood-brain barrier (BBB) is the major challenge for the treatment of various central nervous system (CNS) diseases, because 100% of large-molecule drugs and 〉98% of small-molecule drugs do not cross the BBB essentially. So how to make CNS drug cross the BBB into the brain parenchyma and have therapeutic effects becomes the key factor. Several strategies which enable transport into the brain via endogenous BBB transporters have been developed and tested in experimental and clinical research. Among them, pharmacological strategies such as the use of receptor-mediated transport system and carrier-mediated transport system and nanotechnology-based approaches appear to be the most promising methods. These recent discoveries and new strategies will provide new insight into the drug delivery across the BBB and hopefully lead toward successful treatment to patients with CNS diseases.
