摘要
目的探讨超声造影定量分析动脉相血流灌注参数在肝局灶性结节增生(FNH)和肝细胞肝癌(HCC)鉴别诊断中的价值。方法经肘静脉团注声诺维(2.4ml)造影剂后利用时间-强度曲线分析软件对17例FNH19个病灶(FNH组)和41例HCC41个病灶(HCC组)的动脉相血流灌注参数(到达时间,峰值时间和峰值强度)进行测定,并计算出增强斜率和增强时间,对这些参数进行对照分析。结果(1)造影剂到达时间:FNH组为(10.44±1.43)s,HCC组为(12.64±2.30)s;(2)峰值时间:FNH组为(16.64±2.75)s,HCC组为(21.32±3.86)s;(3)增强时间:FNH组为(6.19±1.79)s,HCC组为(8.67±2.62)s;(4)增强斜率:FNH组为(5.48±1.71)dB/s,HCC组为(3.79±0.99)dB/s。FNH组与HCC组造影剂到达时间、峰值时间、增强时间、增强斜率比较差异有非常显著性(P=0.000);(5)峰值强度:FNH组为(31.42±3.15)dB,HCC组为(30.73±3.86)dB,两组比较差异无统计学意义(P=0.344)。结论超声造影定量分析法可以发现FNH和HCC动脉相血流灌注差异,有助于FNH与HCC的鉴别诊断。
Objective To evaluate the value of quantitative analysis for blood perfusion in arterial phase of contrast-enhanced ultrasound in the differential diagnosis of focal nodular hyperplasia (FNH) and hepatocellular carcinoma (HCC). Methods Each subject was administrated with a bolus injection of the ultrasound microbubble agent SonoVue (2.4 ml) into an arm vein. The seventeen patients with diagnosed FNH (FNH group) and 41 patients with diagnosed HCC (HCC group) were analysed with indexes of blood perfusion. The arrival time (AT), time to peak (TFP) and peak intensity (PI) of the liver lesions were measured by time-intensity-curve software. The enhancement time (ET) and ascending slope (AS) were calculated with these parameters. Results (1) AT: FNH group: 10.44±1.43 s, HCC group: 12.64± 2.30s; (2) TTP: FNH group: 16.64±2.75 s, HCC group: 21.32 ±3.86 s; (3) ET: FNH group: 6.19 ± 1.79 s, HCC group: 8.67 ±2.62 s; (4) AS: FNH group: 5.48 ± 1.71 dB/s, HCC group: 3.79 ±0. 99 dB/s. All parameters showed statistical significance between FNH and HCC groups ( P 〈 0.001 ) ; (5) PI: FNH group: 31.42 ±3.15 dB, HCC group 30.73±3.86 dB; No difference was observed between two groups ( P 〉 0.05 ). Conclusions The quantitative analysis of blood perfusion in arterial phase of contrastenhanced ultrasound can be benefit for the differential diagnosis of FNH and HCC.
