摘要
目的利用B超在颅脑手术中探查病变并导向手术,探讨术中B超在神经外科手术中的应用价值。方法颅脑手术开颅后,在硬脑膜外或硬脑膜下以B超定位,探查病变的位置、轮廓以及与脑室等结构的关系,手术切除病变。对术中急性脑膨出,疑有多发性颅内血肿者实行术中B超检查。结果B超辅助下手术19例。胶质瘤2例,脓肿2例,脑膜瘤1例,血肿14例。B超引导下血肿清除术14例,B超辅助下准确切除病变5例。病灶定位准确,不需探查,切除满意,无脑组织损伤,未出现相应明显的神经系统障碍。结论B超对于皮层下囊性或边界清楚的病变以及功能区病变,具有明显的定位优势。术中B超对多发颅内血肿能准确定位,对明确术中急性脑膨出的原因有重要的意义。
Objective To introduce the use of B - mode ultrasound in neurosurgical operation. Methods Combined with preoperative image data, we used B - mode ultrasound probe to find brain lesion after craniotomy, and resectcd the lesion. The patients skeptical of multiple intracranial hematomas for acute encephalocelc in surgery were monitored by B - type ultrasound. Results 19 cases were examined in the course of operation. 2 cases with brain glioma, 2 eases with brain abseess, 1 case with meningioma. 14 eases with intraeranial hematomas at different intraeranial positions. 14 patients with intracranial hematomas were operated under the navegation of B - type ultrasound. Total lesion resection was achieved in 5 cases. Localization of the lesions was accurate. Exploration was not necessary, injury of normal tissue was quite slight. There are no disturbances of CNS function observed after surgery. Conclusion We can get the real - time positon and image of brain lesion with intraoperative ultrasound. B - mode ultrasound is helpful for total resection of lesion and decreasing the side effect injury. B - type uhrasound is accurate at the navigation and localization of muhiole intracranial hematomas in operation, key to identifying the causes of acute encephalocele.
