体感诱发电位与直接皮层电刺激在大脑功能区手术中的联合应用及效果评价 被引量:16

Evaluation of effects of combined application of somatosensory evoked potential phase eeversal ( SEP -PR) and direct cortical electrical stimulation (DCES) during surgery in central region

摘要 目的术中联合采用体感诱发电位(SEP)波形翻转与直接皮层电刺激(DCES),通过对照组与实验组肌力、感觉功能变化及切除程度来了解此技术的优越性。方法实验组64例运动感觉功能区占位病人术中联合采用此技术进行监测,与对照组64例比较,以肌力、感觉及切除程度作为评价指标。结果A组12例、B组27例术后出现肌力下降;A组2例、B组1例术后出现感觉减退;A组50例全切,14例近全切;B组42例全切,21例近全切,1例大部切除。结论术中联合采用SEP波形翻转与DCES监测技术,能有效保护运动功能。 Objective The combined application of SEP - PR and DCES during surgery in central region ccould localize the eloquent cortexes. By comparing the variation of myodynamia and sensation with the control group, we could conclude that whether this technique had superiority. Method 64 patients (group A) were performend operation with SEP -PR and DCES to locate the surgery position. By comparing with the control group (group B), evaluate the myodynamia, sensation and resection extent. Result 12 patients displayed myodynamia decreasing in group A Vs 27 in group B. 2 patients displayed sensation decreasing in group A Vs 1 in group B. On postoperative MRI or CT, 50 patients got total tumor resection and 14 patients subtotal in group A Vs 42 and 21 in group B respectively. The remaining 1 patient in group B only got partly tumor resection. Conclusion The combined application of SEP - PR and DCES can protect motor function effectively.
