上颌动脉与颈内动脉搭桥解剖学研究 被引量:7

Anatomical research of bypass from maxillary artery to supraclinoid internal carotid artery

摘要 目的探讨上颌动脉(MA)与颈内动脉(ICA)搭桥的可行性。方法5具成人尸头行双侧解剖,解剖颞浅动脉(STA)、MA、颈外动脉(ECA)、ICA并测量其血管外径;额颞瓣开颅,显露ICA床突上段并测量其外径;在蝶骨上磨-骨孔,测量MA与ICA之间距离以及ECA和ICA起始段到ICA床突上段之间距离,比较所测得数据和移植物行走路径。结果MA血管外径(2.66±0.20)mm大于STA分叉部的血管外径(1.92±0.15)mm。MA与ICA之间的距离(4.69±0.34)cm小于ECA、ICA起始段与ICA床突上段之间的距离(17.76±0.47)cm和(18.11±0.51)cm。结论MA与ICA之间搭桥在解剖上是可行的,具有移植血管短、路径直的优点。 Objective To examine the feasibility of bypass between the second segment of maxillary artery (MA) and supraclinoid internal carotid artery (ICA). Methods Five fixed and perfused adult cadaver heads were carried out bilaterally. After a frontotemperal craniotomy, the superficial temporal artery (STA) and its branches were dissected and their diameters at the bifurcation of STA were measured. The MA was located in the infratemporal fossa, following a zygomatic arch osteotomy , and its diameters were measured. After the sylvian fissure was opened, the supraclinoid ICA was separated and its outer diameter was measured. Extradurally 1 -2 cm lateral to the foramen rotundum, a hole was created in the sphenoid bone with a 4.0 mm tip drill. The length between MA and ICA was measured. After ECA and ICA had been exposed, their diameters lengths between the proximal ECA, proximal ICA and supraclinoid ICA were measured respectively. Results The mean caliber of MA ( 2. 66 ± 0. 20 ) mm, was larger than the mean caliber of STA trunk ( 1.92±0. 15 ) mm. The length between MA and supraclinoid ICA was (4. 69 ±0. 34) cm, which was shorter and straighter than the route between proximal ECA or ICA to supraclinoid ICA ( 17.76 ±0.47 ) cm and ( 18. 11± 0. 51 ) cm separately. Conclusions The MA - to - ICA bypass may be an alternative to STA to MCA bypass as well as to ECA or ICA to supraclinoid ICA bypasses.
