时间分辨荧光免疫法检测乙肝标志物的临床评价 被引量:2

摘要 目的:探讨时间分辨荧光免疫分析技术(TRFIA)检测乙型肝炎血清学标志物(HBV-M)的临床意义。方法:用TRFIA和ELISA法对健康对照组115份血清和感染组210份血清的HBV-M进行测定。结果:用TRFIA和ELISA法检测HBsAg对照组和感染组均无差异;TRFIA检测HBsAb比ELISA法灵敏度高,可检出低浓度及过高浓度的HBsAb;TRFIA检测HBeAg比ELISA法特异性强,ELISA法易出现假阳性;TRFIA检测HBeAb和HBcAb与ELISA法相比对感染组差异有显著性。结论:TRFIA法在检测HBV-M是特异性强、敏感性高,其结果间接反映病毒的感染状况,同时也可为接种乙肝疫苗提供依据。HBeAg和HBsAg可作为监测HBV感染和复制的量化指标。动态监测HBV-M的量能够反映机体对病毒感染的免疫反应情况,可作为观察临床疗效的依据。 Objective :To estimate the clinical value of the time resolved fluoescence immunoassay(TRFIA) in detecting serum markers of hepatitis B virus (HBV-M).Methods:We used the methods of TRFIA and ELISA to measure the HBV-M of 115 serum samples from control subjects and 210 serum samples from patients respectively.Results:There were no discrepancy in control subject and patients using the methods of TRFIA and ELISA to measure HBsAg;TRFIA has a higher sensitivity to HBsAb.TRFIA was able to detect excessively low or high eoneentraction of HBsAb.Also it had a higher speciality to HBeAg, while ELISA had a higher rate of false positive relatively.There was statistical difference between ELISA and TRFIA in detecting HBeAb and HBcAb.Conclusion:TRFIA has a higher sensitivity and speciality in detecting HBV-M, which could reflect the HBV infection condition indirectly and provide a reliable guide on injecting HBV vaccine. Monitoring the developmemt of HBV-M content provides a good reflection on our body's reaction to virus infecting,whieh can be used as the evidence for observing clinical course of disease and curative effect.
