摘要
目的探讨尺桡骨骨折旋转畸形愈合对前臂旋转功能的影响。方法尸体8具,共16个上肢。取6个上肢标本,观测上、下尺桡关节的构成及解剖学特点。另取10个上肢标本,造成桡骨干旋前圆肌止点近侧、远侧骨折,尺骨下1/4旋前方肌近侧骨折,制作不同旋转方向和角度的标本,模拟前臂旋转运动,观测旋转畸形造成前臂旋转功能障碍的解剖学因素。结果上、下尺桡关节是前臂旋转功能的解剖学基础,桡骨以"旋转弓"做旋前、旋后运动。桡骨旋转畸形超过20°即可出现旋转受限,当桡骨旋转畸形超过30°、尺骨内旋或外旋畸形超过10°时,旋转明显受限。结论尺桡骨旋转畸形愈合将造成前臂旋转功能障碍,畸形越大,旋转功能障碍越明显。
Objective To study the influence of the rotational deformities of the radius and ulna on the rotational function of forearm. Methods Eight human cadaver speeimens were used to study the anatomic structures and its characteristics of the forearm proximal and distal radio - ulnar joints. The fractures of radius and ulna were produced in ten other human cadaver upper limb speci- mens. The sites of the fractures were the proximal radio - insertion of pronator teres, distal radio - insertion of pronator teres and ulna, and were made into models with different directions and degrees of rotational deformities to observe the effects of rotational deformities upon the forearm rotational function. Results The proximal and distal radio - ulnar joints were anatomic foundation of the rotational function of forearm. When the rotational deformities exceed 20°, the disturbance of the rotational function of the forearm will occur. The disorder of the interosseous membrane is the main cause of the disturbance of rotational function. Conclusions The rotational de- formities will cause the disturbance of the rotational function of the forearm.
