褪黑素对大鼠急性坏死性胰腺炎合并肾损伤的保护作用

摘要 目的探讨褪黑素对急性坏死性胰腺炎(ANP)肾损伤的保护作用。方法72只大鼠随机分为3组:对照组、ANP组、褪黑素治疗组,各24只。ANP组分3次腹腔内注射6%L-精氨酸(L-Arg)1.5 g/kg建立ANP模型;褪黑素治疗组于首次注射L-精氨酸前30 min腹腔内注射1%褪黑素50μg/kg;对照组同法注射等量生理盐水。各组大鼠于末次腹腔内注射后6 h、12 h和24 h分批处死。检测血清淀粉酶(AMY)、肌酐(Cr)水平和肾脏组织中丙二醛(MDA)、髓过氧化酶(MPO)含量,光镜下观察肾脏组织学变化并进行病理学评分。结果ANP组各时点AMY、Cr、MDA、MPO值均明显高于对照组(P均<0.01),肾脏组织损伤重,病理评分高;褪黑素治疗组各时点AMY、Cr、MDA、MPO值均低于ANP组(P均<0.01),肾脏组织病理改变及评分较ANP组明显减轻(P均<0.01)。结论褪黑素通过抗氧化和抗炎等作用发挥肾脏保护作用。 Objective To study the effects of melatonin on acute necrotizing pancreatitis(ANP) associated with renal injury. Methods Seventy - two rats were randomly divided into control group ( n = 24), ANP group ( n = 24), and melatonin treatment group (n =24). ANP group was given intraperitoneal injection of 6% L - arginine ( L - Arg) 1.5g/kg 3 times to establish ANP model. Treatment group was given intraperitoneal injection of 1% melatonin 501xg/kg one time 30 minutes before the first injection of L - argi- nine. Control group was done in the same way with equivalent dose of normal saline injection. The rats were killed separately on 6h, 12 h, and 24 h after the last intraperitoneal injection. The level of hemediastase (AMY) and serum creatinine(Cr) ,the contents of malondialdehyde(MDA) and myeloperoxidase (MPO) in kidney tissue were detected. The histological changes in kidneys were observed under the light microscope and the renal pathology score was calculated. Results The values of AMY, Cr, MDA, and MPO in ANP group on the corresponding time points were significantly higher than those in control group, respectively( P 〈 0.01 ). Meanwhile kidney tissue injury was more serious and pathology score was higher. The values of AMY, Cr, MDA, and MPO in treatment group were lower than those in ANP group on the corresponding time points (all P 〈 0.01 ), Pathological changes in kidney and score were significantly decreased (all P 〈 0.01 ). Conclusions Melatonin protects the kidneys with antioxidant and anti -inflammatory effects.
