摘要
目的比较Amsel′s标准方法、唾液酸苷酶法、革兰染色评分法对细菌性阴道炎的诊断价值。方法对342例门诊女性阴道分泌物标本采用Amsel′s标准法、唾液酸苷酶活性法以及革兰染色评分法分析。结果342例患者Amsel′s标准法检出细菌性阴道病63例,占18.4%;革兰染色评分法检出68例,占19.9%;唾液酸苷酶活性试验检测出75例,占21.9%。唾液酸苷酶法阳性率与Amsel′s标准法比较,两者差异有统计学意义(x^2=9.2,P<0.01);革兰染色评分法检出阳性率与Amsel′s标准法比较,两者差异无统计学意义(x^2=2.12,P>0.05)。唾液酸苷酶法敏感性为95.2%(60/63)、特异性为94.6%(264/279);革兰染色评分法敏感性为85.7%(54/63),特异性94.9%(265/279)。结论唾液酸苷酶法与Amsel′s标准方法、革兰染色评分法相比具有敏感性高、特异性强、操作简便、设备简单和快速等优点,可为临床诊治细菌性阴道病提供新的参考依据。
Objective To compare the diagnostic values of several diagnostic methods (Amsel's standard method, neuraminidase method and Gram's stain scoring method) for bacterial vaginosis(BV). Methods The vaginal secretions of 342 gynecologic outpatients were simultaneously examined with Amsel's standard method, neuraminidase method and Gram's stain scoring method. Results Among 342 subjects, 63 cases of BV were diagnosed by using Amsel' s standard method(18.4 %) ; 68 cases of BV were diagnosed by Gram's stain scoring method(19.9%) ;and 75 cases of BV were diagnosed by using neuraminidase activity detection method(21.9 %). The positive detection rate of BV by neuramini dase activity detection method was significantly higher than that by Amsel's standard method(X^2= 9. 2,P〈0.01) ;while there was no statistical difference in positive detection rate between Gram's stain scoring method and Amsel's standard method(X^2= 2.12, P〉0.05). The sensitivity and specificity of neuraminidase method were 95.2% (60/63)and 94.6% (264/279) respectively, and those of Gram's stain scoring method were 85.7% (54/63) and 94.9% (265/279) respectively. Conclusion As corn pared with Amsel's standard method and Gram's stain scoring method,neuraminidase method is more sensitive and specific,more convenient, simpler in equipment, and more rapid. It can provide new evidence for clinical diagnosis and treatment of BV.
