摘要
目的探讨长期血液透析留置涤纶环双腔导管的效果及导管相关性菌血症。方法选择我院肾内科自2004年3月至2008年7月进行血液透析的患者,对比以内瘘为血管通路的血透患者与以带涤纶环双腔导管为通路的血透患者的Kt/V值。比较涤纶环双腔导管采用单纯以肝素封管和采用联合肝素与庆大霉素封管两者的导管相关性菌血症的发生率。结果以内瘘为血管通路的血透患者与以带涤纶环双腔导管为通路的血透患者的Kt/V值无显著性差异(P=0.47832)。涤纶环双腔导管的导管相关性菌血症的发生率以采用联合肝素和庆大霉素封管比单纯采用以肝素封管时明显减少(P=0.0001)。结论以涤纶环双腔导管为通路是血液透析比较理想的血管通路。为减少导管相关性菌血症的发生率,可采用联合肝素和庆大霉素封管。
Objective To study the application of long-term cuffed catheter in hemodialysis patients and catheter related bacteria(CRB). Methods Choose the hemodialysis patients from March 2004 to July 2008 in our hospital. Compare the Kt/V value of hemodialysis patients using arteriovenous fistula as long-term vascular access with that using silicone dual-lumen catheter with a Dacron cuff. And compare the incidence of CRB between the use of heparin lock and gentamiein-heparin lock in long-term cuffed cathater. Results There was no significant difference of Kt/ V between HD patients using silicone dual-lumen catheter with a Dacron cuff and those using arteriovenous fistula (P = 0. 47832). The incidence of CRB in using gentamicin-heparin lock significantly decreased than that in using heparin lock in long-term cuffed catheter( P = 0.0001 ). Conclusion Silicone dual-lumen catheter with a Dacron cuff as a long-term vascular access for HD patients is an ideal vascular access. Using gentamicin-heparin lock can decrease the incidence of CRB.
