摘要
目的了解我院临床药师参与外科各科室围手术期预防用抗菌药物的情况，对比分析临床药师对临床合理使用抗菌药物的干预作用。方法收集临床药师参与使用前2005年7～11月外科手术病历746份，参与使用后2007年11月-2008年2月手术病历521份，分别采用用药频度（DDDs）排序法原理对比用药频度，分析用药合理性。结果临床药师参与外科围手术期预防用抗菌药物使用后，其选药、用药时机、用法、剂量、用药方式趋于合理，用药时间大幅缩短；各类手术日趋符合卫生部《抗菌药物指导原则》要求。但仍存在选药广谱、用药疗程偏长、剂量偏大、不必要的联合用药等问题。结论临床药师的监督作用仍需加强。
Objective To review the preventive use of antibacterials in perioperative period of clinical pharmacist intervening all surgical departments of our hospital, and to analyse the intervening effect of their rational use of antibacterials. Methods 746 cases during Jul to Nov, 2005 before clinical pharmacist intervening and 521 cases during Nov, 2007 to Feb, 2008 after intervening were collected from surgical operations in our hospital, and the frequency and rationality of antibacterials use were subjected to statistical analysis using theory of DDDs. Results Choice, occasion, usage, dosage and mode of antibacterials use tend to be rational, the time of antibacterials use was shortened. All kinds of operations were according gradually with the demand of The Golden Rule of Antibacterials issued by Ministry of Health. However, there were still many problems, such as wide drug choice, longer time of drug use, biggish dosage, unwanted united drug use, etc. Conclusions The supervision function of clinical pharmacist still needs to be enhanced.
