摘要
目的:分析产超广谱β-内酰胺酶(ESBLs)大肠埃希菌耐药性,并对其同源性进行研究。方法:对56株临床分离的产ESBLs大肠埃希菌进行药敏结果分析,同时采用重复序列聚合酶链反应技术(rep-PCR),以基因外重复回文序列(REP)为引物,对其基因组DNA进行扩增并分型。结果:产ESBLs大肠埃希菌对亚胺培南、美罗培南、阿米卡星、哌拉西林+他唑巴坦、阿莫西林+克拉维酸的耐药率较低,对第二、三、四代头孢菌素的耐药率较高,达90%以上,对阿莫西林、哌拉西林的耐药率为100%;REP-PCR将大肠埃希菌分为6种基因型(A^F),其中A型含13株,B型有32株,为主要的流行型别。结论:产ESBLs大肠埃希菌耐药现象严重,但对碳青霉烯类保持着高度敏感性;基因同源性分析结果表明我院三个病区存在着两种型别大肠埃希菌的感染流行。
Objective: To analyze the drug susceptibility and gene homology of Escherichia coli isolates producing extended spectrum beta-lactamases in our hospital.Methods:The drug susceptibity was determined to 56 isolates,and their genomic DNA of were analyzed by repetitive-element polymerase chain reaction(rep-PCR)using repetitive extragenic palindromic(REP) primer.Results:100% of strains were susceptible to imipenem and metropenem.92.9%,92.9% and 75.0% were susceptible to amikaxin,piperacillin/tazobactam,amoxicillin/ clavulanate,respectively.But these strains exhibited lower susceptibility to two,third or fourth-generation cephalosporin,lower than 10% of strains were susceptible.All strains were resistant to amoxicillin and piperacillin.There were 6 genotype(A to F)in all isolates with REP-PCR including mainly type A of 13 strains and type B of 32 strains.Conclusion:The drug resistance is serious in E.coli isolates with ESBLs,but they are still highly susceptible to carbapenem antibiotic.In addition,there was the prevalence of two kinds of genotype E.coli with ESBLs in three wards in our hospital.
