摘要
目的:探讨安定对学龄前儿童滴注氯胺酮的药代动力学影响。方法:16例3.5~6岁择期手术学龄前儿童,随机分为两组,Ⅰ组为氯胺酮组,Ⅱ组为氯胺酮加安定组。用反向高效液相色谱法测定氯胺酮血药浓度,并计算药代动力学参数。结果:两组房室模型均拟合为二房室开放模型、药代动力学参数:Ⅰ组T1/2α=(0.15±0.04)h,T1/2β=(2.84±0.44)h,CL=(0.34±0.06)L/kg·h,Vd=(0.66±0.29)L/kg。Ⅱ组T1/2α=(0.14±0.06)hT1/2β=(3.92±0.52)h,CL=(0.26±0.05)L/kg·h,Vd=(0.65±0.07)L/kg。结论:安定使氯胺酮清除半衰期延长。
Objective:This paper was to determine the influence of diazepam on the pharmacokinetics of infused ketamine in pre-school children. Methods: Sixteen patients were randomly assigned to group Ⅰ (Ketamine) and group Ⅱ (Ketamine+Diazepam). After planting trachea cannula on patients. We infused 0.05% ketamine to both groups by 3 mg/kg·h and diazepam (0.2 mg/kg) only to group Ⅱ. The concentration of ketamine in plasma was measured using high performance liquid chromatography. Pharmacokinetic parameters were calculated using 3P87 software composed by Chinese Pharmacologic Socioty. Results: Both groups were of two-compartment open model. The T1/2β and the CL differed between the two group (pl=0.008, p2=0.006); group Ⅰ T1/2α=(0.15±0.04) h, T1/2β=(2.84±0.44) h, CL=(0.34±0.06) L/kg·h, Vd=(0.66±0.29) L/kg·h; group Ⅱ: T1/2α=(0.14±0.06) h, T1/2β=(3.92±0.52) h, CL=(0.26±0.05) L/kg·h, Vd=(0.65±0.07) L/kg. Conclusions: Diazepam can prolong the half-life of Ketamine and decrease its clearance.
