摘要
目的:探讨经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折的临床效果。方法:对26例(39椎)骨质疏松性椎体压缩骨折的患者应用球囊式椎体扩张器对病椎进行扩张,并用骨水泥进行填充,通过疼痛视觉模拟评分法(VAS)对手术前后进行对比,并比较手术前后椎体高度的变化,总结手术疗效。结果:本组病例手术均获得成功,1例患者出现骨水泥椎体前渗漏,2例患者出现椎间盘渗漏,但均未出现神经症状,术后随访3~18个月,平均12.5个月,VAS评分术前7~9分,平均为8.5分,术后0~4分,平均为2.2分。椎体高度较术前明显恢复,椎体前缘平均高度术前为14.28±2.43mm,术后为20.51±2.26mm;椎体后缘平均高度术前为22.18±2.12mm,术后为23.49±2.35mm,椎体前缘恢复率62.6%,椎体后缘恢复率11.3%。结论:PKP技术手术创伤小、手术时间短,可迅速有效地缓解疼痛,并可有效地恢复病椎的高度、矫正后凸畸形,具有很好的临床应用价值。
Objective:To explore the clinical effect of percutaneous kyphoplasty (PKP) in the treatment of osteoporotie vertebral compression fracture.Methods:Twenty six patients (39 vertebrae)of osteoporotic vertebral compression fracture had applied vertebral body balloon-type expander for expansion of the diseased vertebrae,and bone cement filling,through the pain visual analogue score(VAS) compared before and after surgery and the changes in vertebral height and sum up the effect of the surgery.Results:The surgical cases in this group are to be successful,one case of patients with anterior bone cement leakage,2 cases of patients with disc leakage,but all have no neurological symptoms.All patients were followed up for 3-18 months.The VAS score before surgery was 7-9 points,with an average of 8.5 points,after surgery 0 to 4 points,an average of 2.2 points,preoperative vertebral body height significantly more recovered.The average preoperative vertebral body height of the front 14.28 ± 2.43mm,after surgery an average height of vertebral body front 20.51±2.26mm ,the average preoperative vertebral body height of the posterior 22.18±2.12mm,after surgery an average height of vertebral body posterior 23.49±2.35mm,vertebral front 62.6 percent recovery rate,vertebral posterior 11.3 percent recovery rate.Conclusion:The PKP technique has the advantages of little hurt, and short operation time.It can relieve the pain rapidly and effectively, and can recover the height of sick vertebrae effectively and correct the kyphosis.Thus has a commendable clinic applicative value.
