摘要
目的评估湖南省医院感染控制主要措施的执行情况，找出差距。方法随机抽样调查该省122所医院，按统一的调查表格和调查方法，由经过培训的专职调查人员调查临床科室人员洗手的规范性、快速手消毒剂使用、微生物学诊断、抗菌药物使用率、外科围手术期预防使用抗菌药物等内容。结果共75所医院资料合格，重症监护病房、新生儿科、内科病房医务人员洗手合格率分别为68．00％、74．20％、62．93％，各合格率之间的差异有统计学意义（P〈0．01）；快速手消毒剂配备率为34．67％～36．00％，平均为37．33％，各科室配备率之间的差异无统计学意义（P〉0．05）；住院患者标本细菌培养阳性率为36．98％，门诊患者标本细菌培养阳性率为37．36％，两者之间的差异无统计学意义（P〉0．05）；住院患者抗菌药物平均使用率为74．65％，门诊处方中抗菌药物使用率为45．52％，两者之间差异有统计学意义（P〈0．05）；围手术期抗菌药物使用率为25．87％。结论除洗手的规范性较好外，其他措施的执行上均存在较大差距。
OBJECTIVE To evaluate the implementation of intervention measures of nosocomial infection in hospitals of Hunan Province. METHODS 112 hospitals were selected randomly, and investigated by questionnaire. The contents of investigation included： standard hand-washing of clinical health care workers（HCWs）, the use of rapid hand disinfectant, microbiological diagnosis, the usage rate of antimicrobial agents, peri-operative antimicrobial application, et al. RESULTS Data of 75 hospitals were available. The qualification rate of hand-washing of HCWs in intensive care units, neonatal units and department of internal medicine was 68.00%, 74.20% and 62. 93%, respectively. There were significant differences between every two qualification rate（P〈0.01）. The usage rate of rapid hand disinfectant was between 34.67% and 63.00%, the average rate was 37.33%. There was no obvious differences between the departments（P〉0.05）. The positive rate of bacterial culture of samples from inpatients and outpatients was 36.98% and 37.36%, respectively, which there was no difference between them（P〉0.05）. The usage rate of antimicrobial agents in inpatients and outpatients was 74.65% and 45.52% respectively, with a significant difference （P 〈 0. 01 ）. The usage rate of peri-operative antimicrobial agents was 25.87%. CONCLUSIONS Except standard hand-washing, the differences between the implementation of the other measures were significant.
