摘要
探讨胃癌患者血清中肿瘤标志物CA724水平的表达及临床诊断价值。方法采用放射免疫法对57例胃癌患者,26例胃良性疾病及30例正常人(健康助血员)进行了血清CA724水平的检测。结果胃癌、良性疾病及健康对照组血清中的CA724中位水平分别为57(255~1515)U/ml、210(160~263)U/ml和121(091~192)U/ml,胃癌组较良性疾患及健康对照组,均达统计学上的显著意义(P<001)。以37U/ml为界值,胃癌组与胃良性疾患的敏感度的分别为:632%(36/57)与77%(2/26),正常对照组中CA724水平均<37U/ml,胃癌组较良性疾病组及正常对照组的阳性率有显著性差异(P<001)。不同病理类型的胃癌阳性检出率也有差异,其中以未、低分化胃癌患者最高(18/25)72%,浸润伴转移癌(10/14)714%,中高分化胃癌(7/11)636%,其他胃癌如残胃癌、印戒细胞癌等(1/7)143%。结论CA724是一种具有临床诊断参考价值的胃癌标志物。
Objective In order to evaluate the serum level expression and the clinic diagnostic significance of tumour marker CA72 4 in the patients with stomach cancer.Methods We detected the serum levels of CA72 4 in 57 patients with stomach cancer,26 patients with benign stomach disease and 30 normal people(health donor)by radioimmunoassay.Results The mean serum levels of CA72 4 in stomach cancer group,benign stomach disease group and normal group were 5 7(2 55~15 15)U/ml,2 10(1 60~2 63)U/ml and 1 21(0 91~1 92)U/ml respectively.The serum levels of CA72 4 in patients with stomach cancer were remarkably higher than those in benign disease group(P<0 01)and normal group(P<0 01).We suppose serum levels of CA72 4 more than 3 7U/ml was positive.The positive rate in stomach cancer group was 63 2%,and that in benign stomach disease group was 7 7%.All the serum levels of CA72 4 in normal group were less than 3 7U/ml.The positive rate in stomach cancer group was significantly different than those in benign stomach disease group and normal group(P<0 01).Moreover,the positive rate was relevant to the pathologic type of stomach cancer.The positive rate in undifferentiated and poorly differentiated group was the highest(72%).It was 71 4% in invasion and metastasis cancer group,and 63 6% in moderately and well differentiated group.It was 14 3% in other stomach cancer,such as residual stomach cancer and ring cell cancer,Conclusion The tumour marker CA72 4 is valuable in the clinic diagnosis of stomach cancer,and its serum levels vary in the different pathologic type.
