摘要
目的评估自行研制的手法固定劈核器,在小切口非超声乳化白内障摘除术中的运用。方法选择20例Ⅲ~V级硬核白内障患者,在术中通过自行研制的劈核器,固定晶状体核,并使核劈成两瓣,通过4mm左右的切口剜出。观察术后视力、内皮细胞平均丢失、角膜水肿等改变。结果20例患者均能顺利完成劈核,术后1周15例视力0.5以上,占75%;角膜水肿7例,占35%;术前和术后,内皮细胞数平均分别为(2638±405)/mm2和(2455±450)/mm(2P<0.01);角膜屈光度分别为(44.05±1.12)D和(44.50±1.05)D(P<0.05)。结论在小切口非超声乳化白内障摘除术中,固定式手法劈核有利于处理硬核性白内障,是安全、有效的手法碎核方法之一。
Objective To evaluate the application of stable-nucleus chopping in small incision non-phacoemulsification cataract surgery. Methods Twenty cases of cataract with their nucleus being degree Ⅲ or harder were performed.All nuclei had been divided into two parts by stable-nucleus technique and drawn out by lens hook under about 4mm sclera tunnel incision. Results Twenty cases were succeeded to chop.On postoperative one week, uncorrected visual acuity was 0.5 or better in 15 cases(75%); corneal edema was 7cases(35%). On preoperative and postoperative, the average cell density of the corneal endothelium was 2638±405/mm^2 and 2455±450/mm^2 cells (P〈0.01) respectively; the average cornea refractive power was ( 44.05±1.12)D and (44.50±1.05)D (P〈0.05)respectively. Conclusion Stable-nucleus chopping is fit for the management of nucleus with degree Ⅲ or harder and the technique is safe and effective in small incision non-phacoemulsification cataract surgery.
