摘要
目的:探讨用钛网进行颅骨修补术发生并发症的可能原因和防治措施。方法:随访2002年8月～2009年2月施行钛网颅骨修补术病人286例,进行回顾性分析并发症原因,并给予对症治疗。结果:发生并发症40例中,切口感染16例、皮下积液8例、癫痫发作5例、继发出血3例、植片下陷3例、皮下积脓3例、肺部感染2例。除1例发生骨窗处硬膜下大血肿而抢救不及时死亡外,其余病人经处理治疗后痊愈出院。结论:修补术应严格掌握手术指征及时机,对有呼吸道感染者和老年病人,应先控制症状,要严格执行手术规范,认真消毒术野,在悬吊及缝扎硬膜时,避免损伤血管,加强术后抗炎抗癫痫治疗,术毕留置术区胶管引流。
Objective： To analysize certain causes to Post -operative complications in repairing skull defect （ RSD ） by Medical titanium mesh （ MTM ） , To propose control measures. Method： Follow up pations 286 cases doing RSD in my division from August 2002 to February 2009, analysize specific reasons, thereby give symptomatic treatments. Result：40cases of complications occurred in our study,wound infection（ 16 cases） , subcutaneous effusion （ 8 cases） , seizures （ 5 cases ） , secondary hemorrhage （ 3 cases） , graft subsidence （ 3 cases ） , subcutaneous empyema（3 cases）,pulmonary infection（2 cases）. Except 1 case died of bone window large subdural hematoma and not timely emergency treatment, the rest of patients recover after treatments. Conclusion： Repair should be strictly guided and timing in surgery. Before surgery , we have to control symptoms in case of respiratory tract infection and elderly patients, Strictly enforce norms operations, seriously disinfect surgical field,and avoid vascular injury in the dural suturing and suspension, To implement the anti - inflammatory and anti - epileptic treatments, Lay drainage pipe in surgical field.
