摘要
目的探讨磁共振波谱成像在前列腺癌、前列腺增生中的诊断价值。方法对经病理证实的前列腺癌13例、前列腺增生18例进行MRI和MRS分析。选前列腺中间层或病灶最大层面作为感兴趣区,测量其(Choline+Creatine)/Citrate即CC/C的比值。结果13例PCa在T2W1图像表现为外周带的信号减低,其中8例为单侧性,其中4例伴转移,5例为双侧性,其中3例伴有广泛转移,18例BPH在T2W1图像上均显示前列腺移行带和中央带的增大,其中16例表现为局灶性结节状高、等、低及不均匀信号,1例表现为弥漫性体积增大,呈不均匀性信号影,1例以外周带增大为主,呈高信号。MRS显示PCa的Cit峰降低,Cho峰升高,(Cho+Cre)/Cit值升高,而BPH患者中,13例各代谢物波峰变化不明显,5例部分区域Cit峰降低,Cho峰升高,但PCa和BPH两者间(Cho+Cre)/Cit值有显著性差异(t值为2.35,P=0.0350.05)。结论前列腺癌的(Cho+Cre)/Cit值显著升高,MRS有助于前列腺癌的鉴别。
Objective To discuss the diagnostic value of prostatic carcinoma (PCa) and benign prostatic hypertrophy (BPH) by using magnetic resonance spectroscopy (MRS). Methods The MRS finding of 13 cases of PCa, 18 cases of BPH proved by pathology were analyzed. The region of interest (ROI) was set on the middle slice of prostate or the region through the biggest lesion. The ( choline+creatine)/cit rate (CC/C) value of each region was measured quantitatively. Results In 13 cases of PCa, low T2WI signal intensity area in the peripheral zone was observed, 8 cases was unilateral (4 cases had metastasis) and 5 cases was hibateral (3 case had extensive metastasis). In 18 cases of BPH, enlargement of the transitional and central zone of the grand was found on T2WI images, 16 of them presented focal high or equal or low or inhomogeneous signal, 1 case with inhomogeneous signal was asystematic enlargement of volume, 1 case had enlargement of periphery zone which presented high signal. On MRS, citrate peak was obviously declined and choline peak was ascended with the (Cho+Cre)/Cit ratio being significantly dropped in PCa.Then ,in BPH,the peak of every metabolite had not conspicuous change in 13 cases.The other 5 cases presented that citrate peak was declined and choline peak was ascended in partial region. But, there was significant difference between the PCa and BPH(t=2.35, P=0.035〈0.05). Conclusion The ratio of (Cho+Cre)/ Cit in PCa increases significantly. MRS may be useful in the differential diagnosis of PCa.
