摘要
目的探讨RhD阴性供者传染病标志物的实验室检测方法的反应性,为RhD阴性献血者传染病标志物、实验室检测方法的选择提供依据。方法(1)利用酶联免疫吸附试验(ELISA)法对孝感地区232 279例献血者进行血清丙型肝炎病毒抗体(抗-HCV)、人类免疫缺陷病毒抗体(抗-HIV)、乙型肝炎表面抗原(HBsAg)及梅毒抗体(抗-TP)检测,其结果呈阳性的样品进一步用逆转录-聚合酶链反应(RT-PCR)确认分析;(2)利用赖氏法或速率法对献血者血清作丙氨酸氨基转移酶(ALT)检测;(3)利用微板法对献血者作RhD血型筛查,对RhD阴性的标本进一步用美国DBL公司单克隆抗体试剂确认。结果232 279份血清标本抗-HCV、抗-HIV、HBsAg、抗-TP及ALT阳性检出率分别为1.52%(3 540/232 279)、0.004%(1/232 279)、4.20%(9 752/232 279)、0.06%(137/232 279)及2.72%(6 321/232 279);RhD血型筛查的确认阴性率为0.32%(746/232 279)和0.30%(700/232 279)。结论ELISA法、速率法对RhD阴性供者传染病指标及ALT检测无1例漏检,灵敏度高,特异性强,结果准确,可作为献血者传染病检测的实验室方法。微板法检测RhD血型具有快捷、灵敏、结果准确、经济等特点,适合RhD血型的大规模筛查。
Objective To probe into the laboratory resist to HCV, HIV, HBsAg,syphilis,ALT,and the resposivity in the sieving test of RhD blood type, and offer basis to establish the negative blood donorrs infectious disease of RhD,as well as the choice of the experimental method for the blood group. Methods 1. Collect 232279 copies of resist blood donors serum in Xiaogan district, and conduct the test to-HCV, resisting- HIV, HBsAg,syphilis by means of ELISA, and detect it further to confirm the positive sample spreads recording by going against enzymes together with(RT-PCR) ;2. Do test to the blood donors' ALT through hanging on law or speeding law; 3. Utilize the little board law to sieve the blood group of RhD for the blood donors,and confirm the RhD negative samples with U, S.A. Dominion/3iologcals Limited monoclonal antibody reagent. Results The rate of 232279 copies of blood final proof to resiste- HCV,- HIV, HBsAg,syphilis,ALT the positive is 1.52% (3540/232 279),0. 004% (1/232 279), 4.20%(9752/232 279),0.06%(137/232 279),and 2.72% (6321/232 279)respectively; The negative rate of the blood group of RhD after further sieving and confirming is 0. 32% (746/232 279 ),and 0. 30% (700/232 279)respectively. Conclusion The method of ELISA law and speed law to RhD negative infectious disease is of high sensitivity and picularity,and the result is accurate, so it can be regarded as the measuring experiment method of blood donor's infectious disease for the patients. It has swift, sensitive, fruits Characteristics, such as accuracy, economy, etc. so the little board law measures of the blood group of RhD is suitable for the blood group of RhD to sift and check on a large scale.
