64层螺旋CT评价颈动脉易损斑块的研究 被引量:3

摘要 目的探讨64层螺旋CT对颈动脉斑块易损性的评估及临床意义。方法33名经血管彩色多普勒超声检查发现颈动脉斑块的患者进行64层螺旋CT颈动脉成像检查,获取相关数据,应用64层螺旋CT工作站相关的后处理技术对颈动脉粥样斑块进行分析,并与血管彩色多普勒超声查结果进行对比分析。结果66个颈动脉段纳入分析,25个血管段正常,41个有斑块。CTA对颈动脉斑块探测的敏感性为100%(34/34),特异性为82.9%(34/41);在区别钙化和非钙化斑块方面,CTA敏感性为100%(26/26),特异性为89.7%(26/29)。结论CTA能对颈动脉斑块的形态学特征作出精确评价,可作为一种有效的无创性手段对易损性斑块的治疗作出指导。 Objective To evaluate the ability of 64-slice CT angiography (CTA) assessing the vulnerable of carotid artery atherosclerotic plaques. Methods Thirty-three patients undergoing color doppler ultrasonography for clinical indications underwent 64-slice CT angiography for the evaluation of plaque morphology. CT angiography data were evaluated for the presence and morphology of plaque in the carotid artery segments. The results were blindly compared with color doppler ultrasonography results on a segment by segment. Results Sixty-six carotid artery segments were analyzed. Twenty-five segments were normal and forty-one contained plaque. To determine carotid artery segments with any plaque, CT angiography had a sensitivity of 100% (34 of 34) and specificity of 82.9%(34 of 41). To determine plaque morphology as calcified (fibrocalcific and calcific) versus noncalcified (soft and fibrous), CT angiography had a sensitivity of 100% (26 of 26) and a specificity of 89.7% (26 of 29). Conclusion CT angiography can accurately characterize plaque morphology and can be a useful tool in noninvasive evaluation of vulnerable plaque.
