摘要
本文对47例初诊为 AML 的形态学(M)、细胞化学(C)和免疫学(I)资料进行了研究分析。结果 POX^+(>10%)者34例,2例为HAL;POX~±(3~10)者5例;1 例为HAL;POX^-(<3%)者6例,1例 NSE^+/NaF^+,3例表达粒系抗原且对 AML 方案反应好,另2例为 HAL。免疫分型显示35例病人表达粒系 McAbHI_(98)和/或 HI_(115),AML可分为五种免疫亚型:第Ⅰ型粒系祖细胞型(DR^(+/-),POX^+),第Ⅱ型原幼粒细胞型(HI_(98)^+,DR^+/HI_(115)^(-/+)),第Ⅲ型早幼粒细胞型(HI_(115)^(+/-)/DR^-),第Ⅳ型幼单核细胞型(HI_(115)^+,DR^+/HI_(98)^(-/+)),第Ⅴ型慢粒白血病粒细胞危象(HI_(98)^+,DR^-)。47例病人经MCI 联合分析确诊为 AML 者42例,HAL5例。M+C 对 AML 诊断符合率为85.7%,免疫分型的符合率为80.9%。作者认为单纯以 POX≥3%诊断 AML 是不全面的,应结合 MCI 联合诊断 AML。
This paper analyses the morphological,cytochemical and immunological data of 47 pre- liminary diagnosed AML cases.34 cases were POX positive(>10%),5 cases were POX weakly positive(3-10%)and 4 cases were POX negative(<3%)that was positive for NaF and NSE or had expressed myeloid markers.Immunological analysis showed 85.9% (36/47 cases)had expressed HI_(98),HIM_4 or HI_(115). Combine with MCI data,42/47 cases were diagnosed as AML which included 4 cas- es POX^--AML and 5 cases were classified as hybrid acute leukemia(HAL). All AML patients may be classified into five subgroups according to the response of blast to the HI_(98),HIM_4,HI_(115) and HLA-DR.
