摘要
目的:通过减数双侧上颌第一前磨牙矫治成人非拥挤安氏Ⅱ类Ⅰ分类错,探讨其治疗前后硬软组织的改变及意义。方法:选择成人安氏Ⅱ类Ⅰ分类非拥挤错患者21例(男7例,女14例),所有病例均减数双侧上颌第一双尖牙,采用方丝弓矫治技术,对治疗前后头颅定位侧位X线片进行测量并对结果进行比较分析。结果:正畸治疗后上前牙及上唇发生了明显的后退(U1-NA,U1-L1°,↓);颌骨的改变不明显,下颌部分发生了轻微的后下旋转(OP-SN°↑),下前牙有压低并有轻微的唇倾(L1-NB°L1-MP°↑);软组织上下唇均有后退(Z角,NLA↑,上下唇突度减少)。结论:上颌单颌减数矫治非拥挤的安氏Ⅱ类Ⅰ分类错,改变主要发生在牙及牙槽骨。此方法适用于牙性或轻度骨性的非拥挤安氏Ⅱ类Ⅰ分类错畸形的治疗。
Objective：The aim of this study was to investigate the change of hard and soft tissue before and after treatment with maxillary first premolars extraction in adolescents noncrowded Class Ⅱ Division 1 malocclusions. Methods ：21 Class Ⅱ Division 1 malocclusion patients （7 males and 14 females）were selected and treated with the edgewise appliances. The maxillary first premolars were extracted bilaterally. A cephalometric radiograph was taken before and after treatment. The cephalometric data were analyzed. Results：After Orthodontic treatment, The upper lip and upper anterior teeth were retracted obviously（U1- NA, U1-L1°↓）, little effect on skeletal. The mandible was slightly rotated clockwise（OP- SN°↑）. The lower incisors were intruded and slightly flared simuhaneousl y （L1- NB°Ll-MP°↑）. Conclusion：Maxillary first premolars extraction which bring about significant dental and soft tissue changes in dental or mild skeletal Class Ⅱ division 1 patients and can be considered as possible alternatives in select cases.
