摘要
目的探讨膀胱移行细胞癌(BTCC)中细胞黏附分子CD44V4与CD44V6表达的临床意义。方法采用免疫组化方法检测92例BTCC及15例正常膀胱黏膜中的CD44V4与CD44V6表达,并根据随访结果,采用双变量相关分析、COX多因素分析和Kaplan-Meier法进行分析。结果CD44V4及CD44V6在正常膀胱黏膜中不表达,其表达与BTCC的临床分期、病理分级呈正相关;CD44V4的高表达同CD44V6表达呈正相关;CD44V4和CD44V6高表达是BTCC患者预后不良的独立危险因素;CD44V4和CD44V6同步表达比非同步表达或表达缺失有较高的术后复发率和较短的生存期。结论CD44V4及CD44V6在BTCC侵袭进程中发挥重要作用,可对其恶性程度及预后做出较准确的评估,对拟定全面的综合治疗方案有重要的指导意义。
Objective It is to approach the clinical significance of the expression of cell adhesion molecule CD44V4 and CD44V6 in human bladder transitional cell carcinoma （BTCC）. Methods The expression of CD44V4 and CD44V6 in 92 cases of BTCC and 15 cases of normal bladder mucous membrane were detected with immunohistochemical techniques. The fol- low up results were analyzed with double variable quantity, COX multiplicity and Kaplan - Meier methods. Results CD44V4 and CD44V6 had no expression in normal bladder mucous membrane. Their expressions were positively correlated with the clinical stage and pathological grade. The high expression of CD44V4 was positively correlated with the expression of CD44Vr. The high expressions of CD44V4 and CD44V6 were independent risk factors of poor prognosis of BTCC patients. The synchronous expression of CD44V4 and CD44V6 had higher postoperative relapse rate and lower life span than nonsynchronous expression and expression loss. Conclusion CIM4V4 and CD44V6 play a key role in the invasive of BTCC. They can provide more accurate judgment of malignance and prognosis for BTCC. Then the results have a significant indication for working out clinical therapy scheme.
