肝癌不同组织区域ADC值变化规律的实验研究

摘要 目的比较VX2肝癌模型肿瘤不同病变区域的表观扩散系数(ADC)值,进一步探讨MR扩散加权成像(MR-DWI)在肝癌诊断中的价值。材料与方法在CT引导下,对30只新西兰实验兔采用经皮瘤组织块注射法进行种植,制作VX2肝癌模型。分别在种植后第4天、1、2、3周进行MRT1WI、T2WI和DWI扫描。观察不同时间肿瘤中心区、边缘区ADC值差别和变化规律。结果30只实验动物,肿瘤种植成功25只。种植后第4天各种检查方法即可显示肿瘤灶,中心区和边缘区ADC值差异无统计学意义。第1、2周中心区和和边缘区ADC值差异有统计学意义。第3周中心区和边缘区ADC值差异无统计学意义。中心区ADC值在2周前小于周边区,而在第3周高于周边区。结论MR-DWI速度快、简单易行,ADC值的应用将会提高对肝脏疾病的诊断水平。 Objective To investigate the value of MR DWI in diagnosing hepatic carcinoma by observed the ADC values in different areas of rabbit vx2 hepatic carcinoma. Materials and Methods The metastatic rabbit vx2 hepatic carcinoma models were established in thirty New Zealand white rabbits. VX2 carcinoma masses were injected into the liver using percutaneous puncture inoculation method guided by CT scanner in each group. MR T1WI, Z2WI and DWI were performed in all experimental animals at 4 day, 1,2 and 3 weeks after implantation. The ADC values in the center and periphery of tumors in different time were observed and analyzed. Results Of 30 rabbits, 25 were transplanted vx2 hepatic carcinoma. The tumor sites could be found in every MRI sequence at 4 days after the implantation. The ADC values between the center and periphery at 4 days and 3 weeks were no statistical difference. Statistical difference was found in the center and periphery at 1 and 2 weeks after implantation respectively. The ADC values of the center were lower than that of periphery before 2 weeks after implantation, and were higher at 3 weeks. Conclusion MR DWI is a fast sequence in imaging and easily performed. The values of ADC may help discriminate between the necrosis areas and normal areas, which will play a role in surgical operation and puncture biopsy.
