脊柱创伤性压缩骨折影像学表现的生物力学原因分析及鉴别诊断意义 被引量:2

摘要 目的观察分析不同病理状态(创伤性、骨质疏松性、转移性)下椎体压缩性骨折表现的不同征象,从生物力学的角度分析各征象的鉴别诊断价值。方法创伤组23例病人,同时设立两个对照组分别为骨质疏松组(20例)和转移瘤组(21例)。所有病例均经过X线平片、CT、MR3种影像学检查。重点观察每组病例的MR影像学表现,对部分征象在3组中的发生率进行统计学处理。结果观察记录的征象包括骨折类型、椎体后缘后凸情况、椎管变窄、脊髓变性、椎间盘损伤、压缩椎体上下方椎体内异常信号等。除压缩椎体上下方椎体内异常信号在3组间比较差异无显著性意义(P>0.025),其余各征象在3组间比较差异均有显著性意义(P<0.05或P<0.025)。借鉴以往脊柱生物力学研究成果,可以分析出部分征象所反映的生物力学特征。结论不同状态下椎体压缩骨折具有不同的生物力学特征并通过影像学表现出来。综合分析多种影像学征象及其所反映的生物力学特征,可以对椎体压缩骨折做作出正确的鉴别诊断。 Objective To observe different signs of compression fracture under different states(traumatic, osteoporosis, metastatic tumor), reveal different biomechanics characteristic through analysis different signs. Summarize the differential diagnosis value of every sign among different states from view angle of biomechanics. Method All patients be divided into three groups: traumatic group(23 cases); osteoporosis group(20 cases) and metastatic tumor group(21 cases), plain film CT,MR were performed in all cases. Observing dif- ferent signs of every group and making statistical treatment to incidence rate of some signs among three groups. Result The signs by observed and registered include fracture type, post-prominence of posterior border, vertebral canal narrowing, spinal cord degeneration, intervertebral disc injury, abnormal signal inside other body of vertebra. The rates of signs are significant different among three groups(P 〈0.05 or P 〈0.025) except the rates of abnormal signal inside other body of vertebra ( P 〉 0.025). to m ake use of the studied resuits of spine biomechanics, we can analysis the biomechanics feature of some signs. Conclusions The vertebrae compression fracture in different states have different biomechanics feature which can be disclosed by different imaging appearance. The location, shape and interior signal characteristic of post-prominence of posterior border can not only clearly show the biomechanics characteristic of vertebral body, but also become a new method which be used for differential diagnosis in different type vertebrae compression fracture.
